Action of Plan

Date: March 1, 2012 Week and Day: 1st week, day 1 I. Area of concern:
The groups area of concern is at Sta. Mesa's, Ezperanza Health center and Tondo Manila.

II. Objectives:
In the morning our group's Objectives is to attend an orientation regarding our duty, to know where our duty will be held and what are the things to be done and not to be done.

III. Plan of Act:


Our group is suppose to be at the health center around 8 o'clock in the morning for the said orientation. After the orientation the group will be going to the designated areas of concern for the courtesy call and to take rounds around the area.

IV. Target:
The groups goal for the day is to be attending an orientation and after the orientation the students will know what is each and everyone's role throughout the duty. The student should strictly know the rules and regulation of the city or community before entering. After the actual orientation the students should go to their assigned community's health center to have a courtesy call and to also have a short introduction of the staffs and what should be done on specific.

V. Resources:
There will no be used resources for the goal of the day. Only attendance of the students will be needed.

VI. Evaluation:
After finishing the goals of the day the students must be able to know where is their area of assignment. The students must be preparing activities depending on the needs of the community. The students must observe and adapt or adjust on the behavior and practices of the community.

Narrative Report
Date: March 1, 2012 Week and Day: 1st week, day 1 I. Objectives:
Our Group had attended the orientation about their duty. They were oriented about the place, which is Manila and was thought about the things to expect and not to be expected from the community. The students was oriented and was assigned to their area of concern which is at Tondo,Manila. After the orientation the students went straight to their assigned area and had a courtesy call and also had a tour at the area. During the courtesy call the barangay helpers and council as well as the community health workers were introduced to the group and once again was oriented in specific details. The group was given several task to be done for the community.

II. Enchancing Factors:


Our Group discussed about preparing activities that would make a big impact in learning and that would also help the community. The group was divide into three smaller groups and in each group came up with different activities daily for two weeks that should be productive and helpful for the community.

III. Determining Factors


Our group had limited participants for the activities. The students at first were afraid to explore around the area because the place was know to be quiet unsafe because of the strong and aggressive personalities of the people in the community.

IV. Solution
Our group went around the area several times with the barangay helper's which made them more relax and felt were safe in the area. The students were more comfortable to perform different kinds of activities in the area.

VI. Evaluation:
The day ended and the students we able to know know their roles in the community. The group understood the situation of the community. Since the students understood the situation of the community they were more encouraged to plan activities that would help the community the best.

Action of Plan
Date: March 2, 2012 Week and Day: 1st week, day 2 I. Area of concern:
The groups area of concern is the people of Barangay 107, San Jose streets Tondo, Manila.

II. Objectives:
The group will be serving the barangay with free check up of each and everyones Blood Pressure for the adults and will check and take note the weight of the children of Barangay 107. For the barangay to know their Health status. The group will also conduct a house to house interview in particular assigned houses to formulate a family nursing problem and to help them make a solution to it.

III. Plan of Act:


The group will perform free check up of Bloop pressure and weighing of the children in the morning staring from 9:00 am down to 11:00 am. Then in the afternoon the students will be conducting an house to house interview.

IV. Target:
The groups goal for the day is to be perform free check of blood pressure to whoever wants to have their blood pressure checked. It will be open for all ages starting from 20 years old and above. The group will also check and take note of the weight of the children in the area. This activity is done for the community, for them to be knowledgeable regarding their health status weather they are candidates for hypotension and hypertension for the adults and weather the children of barangay 107 are underweight or overweight for them to be alert and to watch over their health. After the morning activity the obtained data will be forwarded to the community's health center.

V. Resources:
The group will be providing their own Stethoscope and sphygmomanometer to be used for the blood pressure checking and also two weighing scale to be used for the weight checking of the children.

VI. Evaluation:
After finishing the goals of the day the students must be able to obtain data that would be forwarded to the community health center. And the students must also provide appropriate health teaching according to the patient's current status.

Narrative Report
Date: March 2, 2012 Week and Day: 1st week, day 2 I. Objectives:
The group had successfully done their planned activity. The students collected the data of blood pressure and while getting their blood pressure the students provided health teachings on how to prevent further complications of the patient. The other half of the group got the data of the children current weight and found out that most of them are underweight.

II. Enchancing Factors:


Our group have provided appropriate and very helpful health teachings to our client. We have based our health teaching base on the clients current condition. It is a case to case health teaching. We believe that it would be effective because as much as possible we have given theam attainable health teachings.

III. Determining Factors


The participants of our activity was expecting medications for their hypertension. Unfortunately we couldn't provide it for them. Our group shared effective health teachings in exchange to the absence of what they are asking for.

IV. Solution
We have advised the participants of the program to visit the health center and have a check up. They may have the medications needed for their condition.

VI. Evaluation:
The day ended and the students we able to render a successful program. They were able to help the community. The group had given added knowledge to the community in making a better lifestyle and to improve their health status.
