SULAKIMAN, witness for the Plaintiff, is executed to serve as his testimony in the instant case. This Judicial Affidavit is being offered to prove: A) All the allegations in the Complaint including all annexes appended thereto and which were already marked as exhibits during the Pre-Trial of this case; B) All other related matters, facts and circumstances relevant and material to this case.

This Judicial Affidavit was taken at the office of Atty. Nikki Albert B. Tarucan at Unit 7827, RCB Tower, 108 Legaspi St., Legaspi Village, Makati City. Questions were propounded by Atty. Nikki Albert B. Tarucan and these questions are numbered consecutively and each question is followed by the answer of the witness.

2. Are you aware that you may face criminal liability for false testimony or perjury if you will not tell the truth? Yes I am. 3. Please state your name, age address and occupation?

I am PO3 JULKABRA A. SULAIMAN 32 years old, married, and residing at 303 Ilang-Ilang Road, Muntinlupa City. 4. What is your occupation? I am a police officer of the Women and Child Protection Desk of Muntinlupa City Police Station. 5. Yes. 6. How did you know her? On September 7, 2012, around 3:45 P.M., she went to our office crying and in pain because of the alleged beating that she received from her live-in partner. 7. What did she look like when she came to your office and asked for your help? She was crying sir. She had bruises in some parts of her body and was trembling. 8. What did she tell you? She told me that his live-in partner beat her and took away her four-year old son, DANIEL LACDAO y BAYAGA. 9. What did she mean by beat? She said that the accused punched her in the face and in the different parts of her body and threw her in the living room. She also said that the accused poured cold water and threw the glass at her head. 10. What else did she tell you? In addition to the beating she received, the accused also threatened her life by saying PAPATAYIN KITA. While looking for an itak simultaneously. ............... Affiant further sayeth naught. I am Maria Agnes Tolention, 28 years old, single, and residing at N0.2-A Carmena Compound Brgy. Capang, Muntinlupa City. 4. Do you know the Plaintiff in this case, Natividad Bayaga y Grajo?

Yes. She is my neighbour and best friend. 5. How many years have you known her? We are friends for five years. 6. Where are you on September 06, 2012? I was at home watching television. 7. And then? I heard noises and violent sounds sir. Then I went outside and found such noises coming from Natividads house. And then when I went to their house and shocked to see her bruised, bloodied, and so hurt lying on the floor. 8. Are you also friends with her live in partner, Danilo lacdao y Leona, the accused in this case? Yes sir. He is the live in partner of my best friend for 5 years. They also live in a house near mine. ............... Affiant further sayeth naught.
