Guidelines on how to complete your DBS form 2013-14

Why do I need a DBS check? This is a check that we must complete for all of our mentors you will not be able to enter the school without it. If you already have a recent Enhanced DBS Check for working with children (within the last 6 months), let us know and we may be able to use that. How do I complete it? Come to the ReachOut office in London or Manchester between the hours of 9am to 5pm weekdays. You may be able to collect and complete the form there and then or you can take it away to complete elsewhere. Forms will also be available when you attend training sessions. What do I need to bring? We will need to see THREE FORMS OF ORIGINAL ID from the table below. What if I dont have three forms of ID? Proving your address In section (b) of the form you are asked to fill in your current address. We must see proof of this address. If you do not have any proof of your term time address WE CAN accept proof of your non-term time address. Most UK residents bring: Passport, Birth certificate, Driving Licence and bank statement. Most Non-UK residents bring: Passport, EU National ID Card, bank statement from a UK Branch, Utility bill, letter from University.

You must bring at least ONE of these: Current valid Passport Biometric Residence Permit (UK). Current Driving Licence (UK) (Full or provisional) Isle of Man /Channel Islands; (Photo card only (a photo card is only valid if the individual presents it with the associated counterpart licence; except Jersey). Birth Certificate (UK and Channel Islands) issued at the time of birth

You must also bring TWO of these: Current UK Driving licence (old style paper version). Current Non-UK Photo Driving Licence (valid only for applicants residing outside of the UK at time of application). Birth Certificate (UK and Channel Islands) issued after the time of birth. Bank/Building Society Statement (UK or EEA) (No longer than 3 months old) Bank/Building Society Account Opening Confirmation Letter (UK). Credit Card Statement (UK or EEA) (No longer than 3 months old) EU National ID Card.

Utility Bill (UK) Not Mobile Telephone. (No longer than 3 months old) P45/P60 Statement (No longer than 12 months old) Marriage/Civil Partnership Certificate (UK and Channel Islands). Adoption Certificate (UK and Channel Islands). Mortgage Statement (UK or EEA) (No longer than 12 months old) Council Tax Statement (UK & Channel Islands). (No longer than 12 months old) Letter of Sponsorship from future employment provider (Non-UK/Non-EEA only valid only for applicants residing outside of the UK at time of application). Cards carrying the PASS accreditation logo (UK and Channel Islands). Letter from University (only used in exceptional circumstances when all other documents have been exhausted).

What next? 1) We send the completed form to the DBS and in due course they will send a Disclosure Certificate to the address you completed in section (b). If this is NOT your term time address you MUST make arrangements for someone to forward the ORIGINAL Disclosure Certificate to your term time address. If possible, please ask them also to scan or photograph the original and send a copy to you by email. 2) Please send a copy of the scan to John ODonnell at johnod@reachoutuk.org in Manchester or Frances Blackwell at frances.blackwell@reachoutuk.org in London. 3) Please take a further copy of the ORIGINAL Disclosure Certificate when it arrives and bring the ORIGINAL AND THE COPY to show your project Manager. We will keep the copy and hand the original back to you. You do not have to show us your Disclosure Certificate but if you do not do so you will not be allowed to continue on the project. 4) If there are any relevant entries on the Disclosure Certificate we will normally discuss this with you confidentially to decide whether we are happy for you to continue on the project and we will then have to discuss this with the school where your project takes place. We will not disclose your name to the school. This is to make sure that, as well as us, the school is happy for you to attend the project.

5) You can apply to join the update service offered by the DBS at the same time as you apply for a DBS check with ReachOut. You WILL have to pay 13 per annum for this service (which enables other employers to check your DBS status without the need to do a separate DBS check with the other employer.) This may help you if you volunteer or are paid to work on any other employment which involves working with children. This will save us time and will mean you do not have to complete the above process.
