ENDOCRINE AND REPRODUCTION MODULE StudentLearningNotes Menstrual History Taking

Learning Objectives Knowledge Component: The student should know and reproduce: 1. Elements of menstrual history. 2. Meaning and understanding of the special terms used in history taking. 3. Urdu translation of all the terms used. Skill Component: The student should be able to perform the following task: 1. Apply principles of communication skills to history taking: Establishing rapport i.e. meets, greet, take consent. Building the relationship i.e. showing respect. Understands patient perspective i.e. patient concerns. Providing structure to the narrative of the patient, i.e. taking other aspects of history.

2. Obtain details of all the essential elements of history 3. Summarize at the end and thanks the patient. Attitude Component: 1. Student should demonstrate the positive attitude toward learning. 2. Maintaining discipline in session and behavior with teacher, simulated patient and each others.

Menstrual History
The terminologies used in taking menstrual history, their meanings and proper way to evaluate / inquire about the same. Menarche: Age at which the first menstrual cycle commence. Maahvari kab shuro huey Duration/ cycle: Duration: How many days does she bleed in a Cycle? Cycle.Duration between the 1stday of two consecutive menstrual cycles i.e. if she bleeds once every month, then it is a 30 Days cycle, if bleeding restarts after 22 days, then it is a 22 days cycle. Obstetrical History Taking

Obstetrical History
GravidaTotal number of pregnancies, irrespective of outcome. Aapka kitney martaba hamal thehra hai Para..Records the number of live births irrespective of gestation, or of stillbirths that have Reached 24 weeks. Aapka kitney martaba hamal thehra hai. Issmey sey kitney martaba aapka chatey (sixth) maheeney ya usskey baad bacha paida hua. Blood Group of Patient. Aapka khoon ka group kya hai The Obstetric Past History This records details of every pregnancy, in chronological order. 1. Date of Delivery and delivery gestation / History of Miscarriage & D&E.Ask about every pregnancy and outcome in chronological order. Aapka akhri hamal kab hua tha; kya purey din par bacha paida hua; ya bachaz aaya ho gaya. 2. Antenatal Problems ( preeclampsia/eclampsia, preterm labour, Diabetes, Hypertension )
Kya aapko hamal key doraan kabhi koi peycheedgi hui thi. Agar hui to sugar ki bimari; blood pressure ka barhna; bachey ka waqt sey pehle paida ho jaana

3. Length of labour and whether spontaneous or induced: Bacchey ki dardein kitni der raheen; dardein khud sey shuro hoeen ya drip/dawa key zariye. 4. Type of Delivery. (whether spontaneous vaginal or assisted or caesarean delivery ) Bacha kaisey paida hua; operation sey yaa normal 5. Any complication during labour/ puerperium. Bachey ki paidaaish key duraan koi peycheedgi paish aye yaa normal hui 6. Weight of baby/ Neonatal complications: distress, Jaundice, NND. Bachey ka wazan kya tha; paidaaish key baad bachey ko koi takleef paish aaye; jaisey key saans lene me takleef; peeliyaa; ya phir maut ho gai 7. Age of baby/child now. Any salient feature i.e. learning difficulties, cerebral palsy. Agar kissi bachey ko paidaish key duraan peycheedgi hui; uss bachey ki ab kaise sehat hai; aap key sab bachon ki sehat sahee hai; koi bachey ko kissi qissam ki peycheedgi hai ya nahi.

Pelvic Examination
Introduction The students will learn to insert Cuscos vaginal speculum and to perform bimanual vaginal examination. Objectives Knowledge Component: The student should know and reproduce; 1. Elements of the steps to be taken to insert speculum in vagina. 2. Meaning and understanding of the special terms used. 3. Urdu translation of all the terms used. Skill Component: the student should be able to perform the following task; 1. Apply principles of communication skills: Establishing rapport i.e. meet, great, take consent. Building the relationship i.e. showing respect. Understands patient perspective i.e. patient concerns. Providing structure to the narrative of the patient, i.e. takingother aspects of history.

Pelvic Examination
Perform a pelvic examination including the ability to; 1. Insert a speculum Check for vaginal infection / discharge. Cervical erosion / polyps 2. Take appropriate cultures and cytology specimens from the vagina and cervix. 3. Do a bimanual examination Uterine size, consistency, regularity, position, mobility. Check for adnexal masses and tenderness. Recto vaginal examination.
