SUPPLEMENTAL MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS: This contract made and entered by and between: The Department of Social Welfare and Development - Field Office I, with office address at Quezon Avenue, San Fernando City, La Union herein represented by Director MARCELO NICOMEDES J. CASTILLO, hereinafter referred to as the First Party; -andGIOVANNE R. DUCUSIN, of legal age, Filipino and with residence address at 135 Baccuit Norte, Bauang, La Union, hereinafter referred to as Second Party; -WITNESSETH that1. The FIRST PARTY shall grant additional Cost of Services to the SECOND PARTY in the amount of P20,000.00 in recognition of his/her contribution in the implementation of programs and services of the Department;

2. Any task consequence arising from the said increase shall be shouldered by the SECOND PARTY. 3. Nothing in this AGREEMENT shall bind the FIRST PARTY to grant similar benefit to the SECOND PARTY in the event of the renewal of Contract of Service.
