What is the HealthMapper?


The HealthMapper is an interactive information and mapping system designed for public health managers and decision-makers who need easy access to a database, reports, graphs, tables, spreadsheets, and maps. It was developed by the World Health Organization (WHO) to simplify the use of Geographic Information Systems (GIS) and mapping for public health activities, and to help organize the collection, storage, retrieval, management, and analysis of public health data, at national and global levels. The mapping and information package consist of:

a database of core baseline geographic, demographic and health information; a data management facility; a mapping interface.

It can support a range of roles in public health decision-making including:


assessing resource allocation (health services, water supply, schools); determining geographic distributions of disease; identifying the relationship and correlations; supporting planning and targeting of interventions; strengthening data collection, analysis and management; monitoring diseases and interventions over time.

The system is currently in operation to support a range of public health programmes in over 60 countries in all regions of WHO. It is increasingly being implemented at district levels to support multi-disease surveillance activities.

What is the ART monitoring extension?


The ART monitoring extension in HealthMapper has been developed for district, regional and national ART coordinators, health information personnel and management teams to facilitate data aggregation from the facilities to the district or region and finally to the national level. The ART monitoring extension allows the user to enter and revise data; create reports; and export or import data (see the Main Menu interface below). The

data entry interface is an exact replication of the generic WHO cross-sectional report and cohort analysis forms (see Participant Training Manual for HIV care/ART Patient Monitoring). Similarly, the data reporting output for the cross-sectional form mirrors the generic paper form. There are several data reporting output options for the cohort analysis which follow closely the format of the generic paper form. The HealthMapper ART monitoring extension can be integrated into any pre-existing or future electronic systems at any level..

Training requirements
Training covering the full scope of tools in HealthMapper may be done in conjunction with or separately from the training specifically for the ART monitoring extension. Participants should have access to a computer that has been installed with the country-adapted version of the ART monitoring extension and HealthMapper.

To open the ART monitoring extension of the HealthMapper, first install the HealthMapper on your machine, as per the HealthMapper quick installation guide. Once the HealthMapper is installed, open the HealthMapper DataManager and click on HIV/Care Monitoring extensions. In the HIV Care/Monitoring extensions window, click on HIV Care / ART Monitoring Extension.

The data entry and revision component allows users to enter data directly from the paper cross-sectional reporting and cohort analysis forms, cell by cell, into an electronic database with a similar interface. The data entry screens were created to replicate the generic WHO paper crosssectional and cohort analysis forms. The user will enter data directly from the boxes on the paper forms into HealthMapper. Error messages will prompt users to fill in missing data, and validate proportions, percentages and totals.

Country The country should be preselected. Make sure the country is correct. Region/District Select the first administrative level. Zone/County Select the second administrative level (if applicable). Health facility select the health facility. If the health facility is not in the drop down menu, it can be created using the DataManager. (see Administration Guide).

Fill out this column for each reporting period

If some or all health facilities are missing, you need to define them at this step. To do so. start the HealthMapper DataManager (you should have an icon on the desktop), go to the menu Update Baseline data/Update geographical point/Health Facilities. You will see this screen:

Click on New, enter the name of the facility, click OK and then assign a level to it. You have to assign a level to each facility to be able to see it in the HIV care/ART Monitoring Extension (by clicking the "Assign/Change level" button).

If you need to create maps with point on the HealthMapper, you also have to fill in the latitude and longitude fields (Lat/Long).

From month ___ of year ___ select the month and year of the reporting period. To month ___ if your cross-sectional report is quarterly, select the month and year of the beginning of the quarter you are reporting on. HealthMapper will automatically generate the last month and year of the quarter for you. Date of completion of the form (dd/mm/yyyy) Copy date of completion of form in correct date format from paper form. Person who completed the form Copy name of person who completed the form from paper form

HIV care (non-ART and ART) new and cumulative number of persons enrolled

If you have cumulative data from prior reporting periods, you may enter it in this column, and it will automatically appear in the following reporting period.

Cumulative number of persons ever enrolled in HIV care at the facility at beginning of the reporting period Please check that the numbers in this column correspond to the numbers on your paper form. You cannot enter data into this column as the numbers are automatically generated based on entry from previous reporting periods. This column will contain zeros for the first ever reporting period. New persons enrolled in HIV care at the facility during the reporting period Copy numbers in this column directly from the paper form from the facility. The total will be automatically generated. Double check that the numbers including the total are correct. Cumulative number of persons ever enrolled in HIV care at the facility at end of reporting period This column is also automatically generated by adding the previous two columns. Please double check that the totals in this column correspond to those on your paper form for the facility. Unspecified age or sex This row is intended to count patients who have no assigned age, sex or pregnancy status. Cumulative number of persons ever started on ART at the facility at beginning of the reporting period Please check that the numbers in this column correspond to the numbers on your paper form. You cannot enter data into this column as the numbers are automatically generated based on entry from previous reporting period. This column will automatically contain zeros for the first ever reporting period. If you have cumulative data from prior reporting periods, you may enter it in this column, and it will automatically appear in the following reporting period.

Use this row for patients with missing sex, age or pregnancy status

New persons started ART at the facility during the reporting period Copy numbers in this column directly from the paper for from this specific facility. The total will be automatically generated. Double check that the numbers including the total are correct. Unspecified age or sex This row is intended to count patients who have no assigned age, sex or pregnancy status. For example, if a facility has counted 15 people on ART but does not know the age or sex breakdown, you would enter 15 in this row in the New persons started ART column. This allows programmes to count total number of patients ever on ART without having to force them into categories if these data are unavailable. Number of persons on ART and already enrolled in program who transferred into facility during the reporting period and Number of persons who restarted ART during the reporting period, after stopping ART for at least one period Copy these two numbers directly from the paper forms for the facility. Number of baseline CD4+ count for persons who started ART during the reporting period and Median baseline CD4+ count for persons who started ART during the reporting period (if available) Copy these two numbers directly from the paper forms for the facility. Double check that the number baseline CD4+ counts is less than or equal to the TOTAL number of persons who started ART in the reporting period. Double check that all numbers correspond with those on the paper form for the facility. Click on Next to continue.

On the second screen, you will record the 2nd-line regimen numbers.

Copy these numbers directly from the paper forms for the facility. Make sure to scroll down on the second screen to complete data entry, and that the regimens are in the same order as on the paper form. Totals will be generated automatically. Copy these numbers directly from the paper form for the facility. Double check that all numbers correspond with those on the paper form Click on Finish.

Step 5 |Saving the form


When you have finished filling out one form, a pop-up window (see above) will prompt you with one of three choices:

Save the form and go to next period for the same facility. This option will take you to the health facility quarterly report data entry form for the following reporting period for the same facility (e.g., February, 2005 if you have just entered January, 2005). This may be useful when there has been a backlog of forms and you are entering data for more than one reporting period at a time.

Save the form and go to next facility for the same period. This option will take you to the health facility quarterly reporting data entry form for the same reporting period but the next facility (in alphabetical order as it appears in the drop-down Health facility menu.

If there is no next facility, a message will prompt you to select another region or zone.

Save the form and close the entry form. This option will exit you from the Cross-sectional form data entry form and return you to the main menu.

Step 1 | entering data


Country The country should be preselected. Make sure the country is correct. If it is not, go to File on the main menu bar and select Change working country. Select the correct country. Region/District Select the first administrative level. Zone/County Select the second administrative level (if applicable).

ART start-up groups who have reached 6, 12, 24, 36, etc. months during reporting period

Health facility Select the health facility. If the health facility is not in the drop down menu, refer to section 1.8.1 Add Health Facilities in the Administrator's Guide or see section A.1 above. For month ___ of year ___ Select the month and year of the reporting period. This will determine which start-up groups/cohorts will be listed in the following dropdown menu (see below). Start-up group/cohort Select the ART start-up group or cohort for which you would like to enter data. As in the example shown above, Cohort 11 2004 is the group of patients or cohort who started ART in November, 2004. If the period November, 2004 is selected, the dropdown list for Start-up group/cohort will contain all the start-up groups or cohorts that are at Month 0, 6, 12, 24, etc. during November, 2004 and that have outcomes to report. E.g., for the period November, 2004, ART start-up groups 11 2004 (Month 0 or baseline); 05 2004 (Month 6); 11 2003 (Month 12); and 11 2002 (Month 24), etc. will all be displayed (see screen shot above). Remember, the ART start-up group is always in the grey column of the paper cohort analysis form, and the reporting period will be the Month/Year at the top of each column. For example, in the cohort form

below, the first ART start-up group or cohort is 01/2005, but the reporting periods for entering outcomes for this ART start-up group include 01/2005, 07/2005, 01/2006, 01/2007, etc.

As this number does not change over the lifetime of a cohort, it will automatically be carried over from Month 0/baseline to Months 6, 12, 24, etc. Cohort analysis forms. Transfers in and Transfers out Copy these numbers directly from the paper form for this clinic. For Month 0 or at baseline, there should not be any Transfers in or out and these cells cannot be filled in. Net current cohort This number is automatically calculated by HealthMapper by adding Transfers in to and substracting Transfers out from the original cohort. For all Month 0/baseline cohorts, this number will equal the original cohort number. Double check that this number corresponds to the number on the paper form for the facility.

On original 1st line regimen, On alternate 1st line regimen, On 2nd line regimen, Stopped, Died, Lost to follow-up (DROP) Copy these numbers directly from the paper form for the facility. The total of all these outcomes should be equal to the net current cohort (see previous item). Percent of cohort On original 1st line regimen, On alternate 1st line regimen, On 2nd line regimen, Stopped, Died, Lost to follow-up (DROP) These percentages will be automatically generated and show the percentage of each outcome for the net current cohort. Percent of cohort alive and on ART This number will be automatically calculated by adding On original 1st line regimen, On alternate 1st line regimen, On 2nd line regimen and dividing by net current cohort. Please double check that this percentage corresponds to the number you have on the paper form for the facility. CD4 median Copy this number directly from the paper form for the facility. A decision will be made at national level to use either CD4 median or CD4 proportion. Be sure to be consistent in your reporting of one or the other. Scroll down to view the rest of the data entry cells. Proportion of patients with CD4 200 (of those with available CD4) Number of adult patients with CD4 counts 200 (numerator) Insert the number of patients with CD4 200. Total number of adult patients with CD4 counts (denominator) Insert the number of patients for whom CD4 counts were available. Proportion of adult patients with CD4 counts 200 (calculated) This number will be automatically calculated based on the numerator and the denominator entered. It is important to give both the numerator and the denominator in order to facilitate aggregation across facilities and districts. Functional status total This should equal the number of patients who are on any 1st or 2nd line regimen. It will be automatically generated to provide a denominator

for the proportions described below. Please double check that the denominator corresponds to the denominator in the paper form of the facility (Total W+A+B).

Total working, ambulatory, bedridden Insert the numerator or total number of patients working, ambulatory or bedridden into the appropriate boxes. Proportion working, ambulatory, bedridden These numbers will be automatically calculated based on the individual numerators for each functional status and the overall denominator given above. The proportions should add up to one. Number of persons who picked up ARVs each month for 6, 12 months Copy these numbers directly from the paper form for the facility. Please note that HealthMapper will only allow you to enter a number for Number of persons who picked up ARVs each month for 6 months for ART start-up groups/cohorts who are at Month 6, and Number of persons who picked up ARVs each month for 12 months for ART start-up groups/cohorts who are at Month 12. Double check that all numbers correspond with those on the paper form Click on Finish.

Step 2 | saving the form


When you have finished filling out one form, a pop-up window will prompt you with one of four choices: Save the form and go to next cohort for the same facility and the same period When opening the Cohort analysis data entry form, the default period will be the current month and year, and the default cohort will be the cohort with Month 0/baseline of the current month and year. This option will open up a cohort analysis data entry form for the cohort at Month 6 for that reporting period, and then Month 12, and then Month 24, etc. E.g., for June, 2005, the default cohort is June, 2005 (Month 0/Baseline), the next cohort is December, 2004 (Month 6), the next is June, 2004 (Month 12), etc.

Save the form and go to next period for the same facility This option will open up a Cohort analysis data entry form for the next month for the same facility. E.g., if you have just filled out a data entry form for June, 2005, a data entry form for July, 2005 will open up.

Save the form and go to next facility for the same period and the same cohort This option will open up a Cohort analysis data entry form for the same month, year and cohort for the next facility which appears alphabetically in the Health facility drop-down menu. If there is no next facility, a message will prompt you to select another region or zone. Save the form and close the entry form. This option will exit you from the Cohort analysis data entry form and return you to the main menu.

Cross-sectional form
From the main menu, click on Cross-Sectional (Monthly/Quarterly) Form under Data Reporting.

Type This should be predetermined and unchangeable. If the type of report is not crosssectional, close out of the window and start again at the main menu. Select the level of aggregation and the location(s) you want to work with Aggregate country data This should be pre-selected for your country and may not be changed at the country level. Aggregate District data Check this box to view the districts or regions (administrative level 1) for which there are data. Click on the district(s) which you would like to aggregate data for. To select multiple districts, hold down the Ctrl or Shift button while clicking. If you do not select any districts, HealthMapper will aggregate data for all districts in the country with available data in the report. Aggregate County data Check this box to view the counties or zones (administrative level 2) in the selected district(s) for which there are data. Click on the county or counties for which you would like to aggregate data. To select multiple counties, hold down the Ctrl or Shift button while clicking. If you do not select any counties, HealthMapper will aggregate data from all counties in the selected district(s) in the report. In the example screenshot shown above, Uganda also has administrative levels below the county: sub-county and parish. If you would like to disaggregate according to one of these administrative levels, select the desired county, sub-county or parish.

If you do not select any facilities, HealthMapper will aggregate data from all facilities in the selected county or counties in the report. Generate distinct reports (generate one report for each selected location and for the level of aggregation specified) Check this box if you would like to create separate reports for each facility, district or county you have selected and click the Browse button to save these reports to a designated folder. The following window will pop up:

From the main menu, click on Cohort Analysis Form under Data Reporting.

If you do not select any facilities, HealthMapper will aggregate data from all facilities in the selected county or counties in the report. If you do not select 'Disaggregated by cohort', the report will aggregate all baseline (Month 0), 6, 12, 24 month outcomes for all cohorts of the selected facilities (or districts/counties). There will be one column for each duration of treatment (baseline (Month 0), Month 6 and Month 12, etc.). The health facilities for which there are data will also be displayed (Abongomola, Aduku) as in the example below. Disaggregated by cohort This option allows you to view the same aggregated data in two different ways:

Sorted by cohort This option will display data for each individual cohort aggregated at the lowest level selected. E.g., December, 2004 cohort data (across all health facilities) will be displayed for Baseline/Month 0 then Month 6 then Month 12 outcomes, followed by January, 2005 cohort data for Baseline/Month 0, Month 6, Month 12 outcomes. Cohorts are displayed in the order that they began treatment from the earliest to latest dates. In the example below, all 6 month outcomes for all 12/2004 cohorts at Abongomola and Aduku are aggregated in the first column.

Sorted by duration of treatment This option will display data disaggregated by duration of treatment (but aggregated by lowest administrative level selected). Missing data report

This report generates a list of all facilities for which no data have been entered for a given time period. It will not report on facilities for which only partial data have been entered.

From the main menu, click on Missing data report under Data Reporting. Type Select which type of form you would like the missing data report to be generated. Select the date range elect the month and year for the beginning and end of the reporting period you would like to see the missing data report. Click on OK to generate report in Excel or on Close to return to the main menu.

Export and import


The data exchange functions allow you to export and import data in order to report up, or to further analyse the data using another programme such as EpiInfo, SAS or SPSS. You MUST use this function for reporting.

1 |Export

From the main menu, click on either Cross-sectional form or Cohort analysis form under Export under Data Exchange. Select the desired data range you would like the report for, and the level of aggregation by region and zone. Click on OK and select the location you would like to export your data to. The window will look as follows:

From the main menu, click on either Cross-sectional form' or Cohort analysis form' under Import under Data Exchange.

Select the location you would like to export your data to. The window will look as follows:

View last log This will give you a report on the status of your import.
