Turned around in 18 months, for Kindred Care, a 240-bed skilled facility that was the Kansas City Star newspaper as one
of the 13 worst facilities in Kansas City. Consequently accomplished the following 3 items: 1) Increased census from 89
to capacity of 240; 2) Expanded Alzheimer program from 22 to a 3-tiered 48-bed program; and 3) Created a high-end 51
bed private pay amenities wing and a short-term rehabilitation wing with in-house rehabilitation team.

Turned around for Covenant Care in Omaha, Nebraska a 173-bed facility with a census of 129 due to a freeze on
admissions imposed by the Nebraska Department of Health because of 43 previously in-house acquired pressure wounds.
The freeze on admissions was lifted in 5 weeks and the facility filled to capacity in 90 days. Subsequently, within 6
months managed an additional 4 buildings in 2 states, and within 2 quarters increased profit to $430,000 quarterly, with
no clinical indicators below corporate thresholds.

Served as the Senior Administrator at Meritcare, increasing Medicare ADC from 12 to 33 in an 83-bed facility and from
4 to 14 in the 74-bed facility through changes in clinical leadership, aggressive marketing, expanding rehabilitation
programs, which proved instrumental in exceeding established goals by $200,000 in just 1 month.

As Director of Operations at Braswell Family Senior Care increased in 2002 rehabilitation contribution 35% by
consolidating services and negotiating a new vendor contract. Eliminated agency usage company-wide with clear
expectations, revised employee benefits, and aggressive recruitment

Recipient of a corporate award in 2009 while serving as Senior Executive Director with Horizon West, for producing the
best patient survey and Medicare utilization of the 27 skilled facilities within the company structure that culminated in
exceeding established corporate goals by $400,000.

2012-2013 Administrator
SunMar Responsible for 2 facilities: Country Oaks Care Center, 78 beds (40 SA-38 SNF) and Pomona Vista Care Center, 59 beds
(Alzheimers Secured Unit) Increased census to 97% and 99% as a benefit of physician recruitment. Profitability increased by 20% by
though P & L improvement, cost reduction strategies, and improvement of mix. Reviewed labor, census/mix, facility rounds,
incident/accident reports, COC, 24 hour reports, PPS/managed care, and met with department heads on a daily basis.

2010-2011 Administrator
Cambridge, Bell Gardens, Responsible for 135 bed facility increased Medicare ADC to 20 and Subacute to full capacity at 24. Ensured
regulatory compliance through a focus on quality improvement initiatives and leadership team building which lead to an improvement to a
4 Star rating. Oversaw successful REAC/HUD inspection.

2006-2010 Senior Executive Director
Horizon West, Southern California, 2006-2007 responsible for 5 facilities 400 beds Initial focus: Survey preparation and subsequently
census growth. In 2008 the primary focus became the oversight of the construction of a new 99 bed skilled facility, which was the
objective at the time of hire while maintaining current operation. Received awards for the best survey and Medicare utilization of the 27
skilled facilities in the company and beat budget by $400K in 2009. 2010 completed survey 4 months early.

2002 - 2005 Director of Operations
Braswell Family Senior Care, Southern California, 12 facilities 1100 beds
Improved the rehabilitation contribution by 35% through consolidation of services. Oversaw the election of 12hour shifts,
the development of a employee handbook, improvements in benefits, which all led to the elimination of agency use and a
dramatic reduction in OT. During my tenure AR and clinical support was brought in-house.

2001 - 2002 Senior Administrator
Meritcare, Northern California, 2 facilities, 157 beds
Built and lead high performing teams able to enhance the clinical skills of the team. Developed relationships with key physicians and
referral sources. Expanded rehabilitation programs. This all lead to the increasing Medicare from 12 to 33 ADC in one facility and 4 ADC
to 14 ADC in the other. The budgeted profit exceeded budget by $200 K in one month alone.

1999 - 2001 Director of Operations/OPS Leader
Covenant Care Corporation, Northern California, 15 facilities, 1630 beds, Midwest, 5 facilities, 616 beds
No. CA: Re-defined admission and discharge criteria for the 3 assisted living facilities, which decreased staffing requirements, improved
quality of care, and benefited the associated skilled facilities. Midwest: Eliminated a freeze on admissions due to 43 pressure wounds in 5
weeks after becoming the Administrator of the primary facility while increasing census from 129 to 173 in 90days. Facilities were
exceeding budget by $430K, had no lost time, and clinical indicators were consistency averaging below corporate and national thresholds.

1998 - 1999 Executive Director
Life Care Centers of America, Idaho. 120 beds. Increased census from 85% to 95%, converting a troubled facility into a training site for
DHS surveyors.

1997 - 1998 Regional Manager
Sunrise Health Care Corp., Idaho. 11 facilities, 900 beds. Improved overall census while improving overall clinical indicators.
