On March 5th 1980 I was 16 years old and involved in a life threatening accident which required a tracheotomy to be performed

at the site and I was pronounced dead on 3 separate occasions. I had driven off the road and hit a large oak tree, which caused the car to actually go airborne several feet according to some spectators. The car then caught on fire, but when the fire department arrived they were not immediately able to remove me from the wreck as my feet had become entangled in the break and clutch pedals when the engine was actually forced through the firewall of the vehicle and caused it to ignite. My face came into contact with the steering wheel and of course during that time frame of 1980 (the care was a 1971 Camaro), there were no airbags. I broke every bone in my face and completely crushed my nose as well as a fractured skull. I broke my jaw, lost several teeth, broke both cheekbones, suffered many cuts, lacerations, and burns, and still suffer physically and emotionally in many ways today. I spent several weeks in the intensive care unit at Holyoke Hospital in Holyoke, MA. In another separate incident about 10 years later, I broke my right collarbone when I was tackled by the South Hadley Police Department. I also separated my shoulder, broke a rib, and a small piece of bone was separated from the rest of my collarbone. I then had to ensure many operations and procedures to try and fix those 2 accidents, including extensive plastic surgery and many hours in a dental chair after school when other kids were enjoying sporting events, etc. I eventually ended up with dental implants, which although work fairly well, I had them installed during the time frame when they were a relatively cutting edge procedure. Because of this my results have been less than optimal and occasionally incur extreme amounts of pain which is unfortunately without any warning. On June 28th 1993 due to a drug bust, which I was found guilty and sentenced to a suspended jail sentence as well as felony convictions. Due to an error made at the laboratory and an overly aggressive District Attorney, I was forced into a plea bargain or face up to 3 years in jail. I chose the guilty sentence, which kept me out of jail, but also has impacts on my life today, such as my ability to gain employment in any employment in any type of security position, which is well over 50% of all jobs. This has made it almost impossible to find adequate employment for myself, my wife of 20 years, and my 3 sons. Fortunately, my father owned a moving company and hired me as a truck driver and long distance furniture mover where I did severe damage to my back over the course of approximately 25 years. The business was sold in 2006 and I have been reliant upon my mother, and brother as an oil delivery man but the pain was too severe and the medication too intense for such a safety-sensitive position. My brother and I agreed for me to leave employment for the benefit of both parties. I collected unemployment for approximately 18 months and havent worked since then other than for approximately 2 weeks and found the same problem. Although the company stated it was due partially

to my criminal background as I was awaiting the outcome of the assault charges for assaulting my wifes boyfriend when me and my 13 year old walked in on them having sex on the living room floor. I proceeded to divorce her when I found she wasnt willing to testify in my defense although the statements to be asked of her would require 100% true responses that would have found me 100% not guilty. I then found out of 2 more affairs she was having without my knowledge. She moved out and left me with our 3 kids and within 3 weeks moved in with another new guy. She had begun drinking excessively approximately 6 months earlier. I filed for divorce, sold our house, gave her half the proceeds and she proceeded to get a restraining order preventing me from seeing my own kids even though she had moved out and in with her boyfriend for 10 months before bamboozling the judge into this ridiculous mess. The oldest went and lived at College while the middle went to live with my mom and the youngest with me. I continued to pay her child support of $500 per week plus health insurance coverage even though they didnt live with her and my gross pay was only $625 per week. My ex-wife was able to receive this child support although she was living with her boyfriend at Hampden Beach. The kids all continued to stay living with me as they were aware of her drinking problem progressing. She would tell my 12 year old son she was going out for coffee and return home 5 days later, ultimately turning into 9 months later, before I had to file for divorce. This was told to the judge but the judge had the right to believe her and did, and after being advised of the necessary medications I was having to take for daily pain relief. This continued for approximately 2.5 years while I began therapy for severe depression and chronic pain and just extreme unhappiness. The criminal charges were dismissed after I paid the guy who was having sex with my wife $55,000 to settle even though he was still also married, but the charges remain on my record to this day. I was unable to gain employment due to chronic back and neck pain as well as chronic epididymitis. Also unable to gain employment as my criminal record will prevent me from any job in or out of my field of expertise, which Ive been involved with almost my entire life. After my divorce, I caught my wife of 20 years in bed with another man and assaulted him and was charged with 4 felonies. I was also charged with 4 felonies in 1993, including assault and battery on a police office and drug trafficking. I have attended and completed at least ten rehab facilities but without any positive long term results. I am completely reliant financially on my family members. I have been involved in several auto accidents beginning in 1980 where I lost my teeth and broke every bone in my face as well as fractured my skull, ear, etc. I was treated at Holyoke Hospital but due to the amount of time expired the records are unavailable. I have had extensive dental work performed including implants by Dr. Moini on Liberty St, Springfield, MA and Dr. Marriano in East Longmeadow, MA who both stated it was their most difficult

and challenging case. I was involved in an auto accident in approximately 1995 where my head hit the roll bar of my Jeep and caused a compression injury. I was involved in another auto accident on Thanksgiving day where my 2000 Lexus was totaled and I sustained extensive neck and back injuries which I have been seeing several doctors who have prescribed me excessive amounts of painkillers which would also prevent me from being employed. I have 2 separate shoulders which cause my occasional discomfort, one due to a ski accident and one due to the South Hadley Police Department assaulting me in 1993. My collarbone was also broke at that time and a screw is still holding the bone in place. I have several wires still intact from the facial injuries I sustained in 1980 as my jaw was broken as well as both my cheek bones. I suffer severe migrane headaches still today and tremendous pain due to all my teeth being not only knocked out, but experimental techniques being used to try and fix the problem at the time where they used actual pig gums to replace the gum tissue I had lost. I fractured my left leg while jump starting a friends car in about 1985 as he lurched the car forward while I attempted to hook up the jumper cables. Fortunately, I havent had any adverse effects from that mishap as of yet although I am told arthritis could set in as it has in my left foot and toe from motorcycle accident which the bike was totaled in 2003 but other than a broken foot I was unhurt. I do still limp slightly from that injury today. Also during the fight when I walked in on my wife and the other man, I punched him with my right hand and incurred what is apparently known as a boxers fracture or just simply a broken hand. This bothers me almost every day and arthritis has begun to cause pain. I saw a doctor in Northampton and the following day and can get the information if required. I broke my nose in the auto accident in 1980 and twice since which has caused me a lifetime of difficulty breathing due to a deviated septum. I was operated on it at Mercy Hospital in Springfield, MA in the 1990s (late) or early 2000s but again broke it during a fall in the shower. I still have 2 wires in both separate cheek bones which were installed in order to hold my jaw in place which although sounds very gruesome, is very effective and allows me to eat. Sometime in my late 20s, I noticed I was becoming very tired during the day, regardless of the amount of sleep I had the night before. I went to my primary care physician, who said I probably had mononucleosis and it would go away in time. After several years passed and I struggled to stay awake in school and at work, I was advised to go to a sleep specialist who diagnosed me with narcolepsy, which as I understand it, just means that certain brain cells located in the frontal lobe of the brain are clumped together in a different pattern than the average person. Unfortunately, as time passed, the condition has worsened, almost causing accidents and preventing me from taking any job responsibilities at my

familys moving company. I was treated with several types of medication which acts similar to caffeine or drinking large amounts of coffee. I was several doctors for this, including Dr. Alfred Scruggs in San Diego area, as well as Dr. Mercadante located in Ludlow, MA and Dr. Quaraghi, located in Somers and Hartford, CT. I had a vasectomy in approximately 1995 by a doctor who is now retired on Chestnut St in Springfield, MA who apparently botched the surgery because I have been in chronic pain ever since within the exception of approximately 1 year immediately thereafter. I went to several Urologists who performed many tests and procedures as well as operations as recently as December 2008 or 2009 but to no avail. I suffer from severe pain most likely due to a condition called chronic epididymitis which causes severe pain in the groin or testicals. I went to the Lehey Clinic in Framingham and saw their best Urologist (the head of the department, Dr. Hanaway) who again performed a host of severely uncomfortable tests to no avail. He could only suggest possible surgery to remove the testical also known as an orchadectamy, but he also stated there was only a 50% chance of pain relief and possibly worse pain. I have been dealing with medication since, as the nerve blocks and all other procedures including the T.U.N.A procedure were ineffective. The procedure calls for a metal rod to be inserted in the penis at different locations and extreme amounts of heat discharged, the ultrasound technician stated I had a prostate of a 70 year old man and said there were extreme deposits of calcite and other things I try not to remember. When I am able to pee which is painful, there is no power or strength or pressure behind the urine and sometimes I have to stand there for 20 minutes waiting to be able to pee. I have to get up 2 to 3 times per night to pee again similar to 10 years ago and most of the time I have to start and stop during the process which makes for a messy cleaning process after each urination. I cannot tell you which causes me more pain or discomfort, my chronic back and neck pain or the epididymitis but I can tell you I have worked for 33 years legally and you are welcome to check my records and I am not a lazy man, but the amount of pain I incur on a daily basis has caused me mental anguish as I havent been able to care for my family who has left me and I have become a huge burden to my immediate family. One might ask why not try surgery again as a possible option to rectify these problems. I am a man of 50 years old and have had my fair share of medical procedures. When I was 13 years old I had dental braces installed for almost 3 years. One week after they were removed I got into a severe auto accident and had most of my teeth removed. When I was 17 years old I had a root canal performed by a dentist who had just graduated dental school and he performed the operation on the wrong tooth. When I was 35 years old I had laser vision surgery which has a 99% chance of success. I was one of the 1% whose

surgery did not go as planned and they lost a part of my eye in the machine. I have no more interest in medical procedures or operations and I hope that you would understand my position regarding my opinion. When my shoulder was broken in 1993, my regular doctor who performed the surgery was on vacation and had his assistant fill in to remove the hardware which had been installed to assist in the healing process. Unfortunately, my regular doctor failed to notify or document to the attending physician that he had used a permanent metal screw instead of a metal removable pin which would require removal. During the follow-up visit the attending physician tried insistently and with excessive force to remove the metal screw that had been installed in my shoulder in order to permanently hold the piece of collarbone in place that had been broken free during the accident. After approximately 20 minutes of extensive pain due to the doctor using a vise-grip type of tool to remove a non-removable screw, he referred to his notes or took an x-ray and realized his error at my expense. On another occasion, I was admitted to a hospital in Long Island, NY for a relatively simple procedure that still required the use of having an I.V. installed. In my situation, 5 different doctors and 2 nurses were all unable to perform the installation of the I.V. into my arm, which prevented the operation from happening as planned and costing $7,000 as it was elective surgery. Although I have had many more accidents and operations and broken bones, etc., I have tried to limit it to the most severe as well as the ones I was able to obtain the records on today. The 3 nearest doctors I have seen over the past 18 months due to my relocation from Connecticut to Massachusetts have all agreed with the previous physicians diagnoses and suggest medication as an option if it does seem to have a positive effect and can be tolerated at the extreme doses that I am prescribed. I realize that the amount which I am eligible for are not enough to live in any form of luxury like I have had in the past when my family owned a business. I am hoping it will at least allow me, for a short time, to just live the life of a normal tax paying citizen. For those reasons and others that I would prefer not to disclose or are unable to get documentation from the facilities that botched these procedures, I would hope you could understand my reasons for not wanting to again try the more radical and invasive processes of medicine such as surgery and just continue with what has seemed to work the best so far. Although far from perfect and preventing me from a lot of the sports and other activities such as skiing, martial arts and bike riding, I have weighed out the pros and cons of the potential outcomes of these surgeries as well as my luck associated with them and would prefer to just spend what years I may have left on this planet on as little pain as possible and allowing me to live a lifestyle that is acceptable and

bearable without taking any additional risks of possibly damaging the only body I have been blessed by God to be given. I ask that you consider providing me with disability benefits immediately so I can begin to repay my family and creditors although I have had to declare bankruptcy. I am hoping it will also relieve some of my depression which I know is caused by my unfortunate situation. Please contact me or any of my doctors if you have any questions.
