l 17, 2014 When: Daily from 9 AM 12 PM Location: Livermore Park Cost: $85 (includes camp t-shirt). Proceeds to benefit the Folsom High School Soccer Program. The camp will focus on individual technical skill development along with some basic individual and group tactics. Additionally, each day coaches will cover a special topic, such as nutrition, goal setting and fitness. The camp will be run by the FHS Girls Soccer coaching, including Varsity Coach, Rick Carrigan, and Junior Varsity Coach, Steve Freitas. FHS players will also be out working with campers. This is a release of liability. Read carefully before signing SINCE ADULT SPORTS CAN BE DANGEROUS, THE CITY OF FOLSOM REQUIRES ALL PARTICIPANTS TO ASSUME ALL RISKS BY SIGNING THIS GENERAL LIABILITY RELEASE.
For and in consideration of being permitted to participate in any City of Folsom 2014 activities, I , on behalf of myself, heirs and assigns hereby voluntarily release, discharge, waive and relinquish any and all claims or actions for damages for personal injury, death, or property damage which I may have, or which may hereafter accrue to me, as a result of my participation in any 2014 activities during play and while I am at the facility while others play or for any other reason. This release is intended to discharge, in advance, the City of Folsom, Folsom Cordova Unified School District, Los Rios Community College District, its officers, employees and agents, and the owners, and maintainers of any facility used for the activities, from any and all liability arising out of or connected in any way with my participation in any 2014 activities, even though that liability may arise out of negligence or carelessness on the part of the City of Folsom, Folsom Cordova Unified School, its officers, agents or employees, or the owners or maintenance of the facility. This release is intended to be construed as broadly as possible with the intention that neither the City of Folsom, the Folsom Cordova Unified School District, the Los Rios Community College District, its officers, employees, owners and agents, be held responsible for any injury or death. I further understand and acknowledge that serious accidents and injury occasionally occur during any activities, and that participants occasionally sustain personal injuries, death or property damage as a consequence thereof. Knowing the risks, I have voluntarily applied to participate in the activity and hereby agree to assume those risks and to release and hold harmless the City of Folsom, Folsom Cordova Unified School District, Los Rios Community College District, its officers, employees or agents, and the owners and maintainers of any facility used for the activity, who might otherwise be liable to me for damages. I further understand and agree that this release, discharge, waiver, and assumption of risk is to be binding on my heirs, executors, administrators and assigns. I further agree to indemnify and to hold harmless the City of Folsom, Folsom Cordova Unified School District, Los Rios Community College District, its officers, G.S.S.A., U.S.S.F, A,S.A. employees and agents, and the owners and maintainers of the facilities, from any loss, liability, damages, cost, or expense, which they may incur as a result of any injury or property damage I may sustain while participating in the activity regardless of the cause of such loss, liability, damages, cost or expense.. I have read this General Liability Release, understand that I give up substantial rights by signing it, and sign it voluntarily.
