Please print clearly or type your information.

Section B:

Family Information. List the people in your household. Include: yourself, your spouse (if you have one), and your/your spouses children, if you provide more than half of their support from July 1, 2012 through June 30, 2013. Also include other people if they now live with you and you provide more than half of their support and will continue to provide more than half of their support from July 1, 2012 through June 30, 2013. Do not include foster children in the household. Also write in the name of the college any household member will be attending for at least half time during July 1, 2012 through June 30, 2013.

Section C:

Untaxed Income. You and your spouse must complete this section to report sources of income not subject to federal income tax. DO NOT INCLUDE: student aid, earned income credit, child tax credit, welfare payments, untaxed Social Security benefits, WIA educational benefits, combat pay, benefits from flexible spending plans/cafeteria plans, foreign income exclusion or credit for federal tax on special fuels

Section D:

Part 1 asks whether anyone in the household received benefits from the Supplemental Nutrition Assistance Program (SNAP, formerly known as food stamps) at any time during the 2010 OR 2011 calendar year. (Refer to your answer to question number 96 on your Student Aid Report.) Part 2 asks whether your or your spouse PAID child support in 2011 because of divorce or separation or as a result of a legal requirement. Dont include support for children in your household, as reported in question 93 on your Student Aid Report. (Refer to your answer to question 43b on your Student Aid Report.)

Section E:

Sign, date and return the Dependent Student Verification Worksheet to the OU Office of Financial Aid Services. Keep these instructions and copies of everything you send to the financial aid office.

Complete the next two pages as instructed and return them to Financial Aid Services.

If either I, or if married my spouse who is listed in Section B of this worksheet, paid child support in 2011 indicate below the name of the person who paid the child support, the name of the person to whom the child support was paid, the names of the children for whom child support was paid, and the total annual amount of child support that was paid in 2011 for each child.
