PROCEDURE FOR FILING AN APPLICATION FOR GRADUATION

This information was already in a modified format, the original procedure is posted at west
116, Main Campus.

NEEDED INFORMATION:

A. CLASS CARDS (FOR COMPLETING FIRST YOUR CURRICULUM SHEET)

B. RECENT REGISTRATION CARD
C. RECEIPT FOR THE APPLICATION PAYMENT

STEPS:

1. Fill up Curriculum Sheet at the Chairperson’s office.

a. The grades though incomplete is okay just indicate the name of the professors for
each subject taken. (It will be used for tracing gradesheets).
b. Have it signed by Arch’t. Pabello, Vilma, Chairperson. At the end of each enrolling
semester.
c. Photocopy it, 1 copy only to be passed together with the application.
2. Proceed to the Cashier
a. Get a receipt, Application Fee Php50.00
3. Proceed to W116
a. Present your receipt and current registration card
 b. After presenting you will be given a form to be filled up
i. Option 1, have it filled up first and then photocopy it thrice ((3) times). A total of 4
copies on hand
ii.Option 2, have it photocopied first, ((3) times) and fill it up.

Note: You can ask procedures by yourself

c. Submittals:
i. ON TOP OF IT, THE ORIGINAL APPLICATION FOR GRADUATION STAPLED
TOGETHER WITH THE 3 COPIES OF APPLICATION FOR GRADUATION
AND THE PHOTOCOPIED CURRICULUM SHEET. TOGETHER WITH IT IS
THE RECEIPT OF YOUR 50 PESO PAYMENT FOR THE APPLICATION. AND
TO BE FILLED UP BY THE ATTENDANT, YOUR CLAIM STUB.
