Near miss reported: First Aid Treatment

Important Activities/Event:
On 20-03-2014
At 12:10 Hrs, ST-10 tripped due to T-Controller Network switch failure. Net work switch replaced by I&C as
per approve modification.
ST-10 casing drain valve SSD-1 didnt open when machine tripped.
ST-10 started and synchronized at 23:52 hrs after network switch replacement in T controller and casing
drain valve SSD-1 repair works. SSD-1 position feedback was getting fault alarm due to deviation. I&C
forced the same in order to get start permissive, after start-up same was unforced. SSD-1 potential meter
is defective, spare not available as per EMD.



GT-11, MMD & I&C to attended the hydrogen leaks at collector compartment and at cylinder station.
Hydrogen consumption become normal.
GT-21 offline water wash completed.
GT-21 started, synch at 11:48 Hrs and performance test carried out at 16:30 Hrs.
MED- 224 Distillate drain line near CO2 booster pump suction MOV detached, it was plug from detaching
point by diverting distillate to reject header for 25 minutes.
GT-41 shut down and de-synchronized with the grid at 21:17 hrs for COP strainer removal and post HGPI
performance test.
On 21-03-2014
ST-30 shut down and de-synchronized at 01:18 hrs. GT-33 shut down and de-synchronized at 02:28 hrs as
SEC given clearance at 02:20 hrs for GT-33 shut down to attend HRSG-33 main steam pressure wqualizing
valve 11807.
GT-33 forced cooling carried out, isolation carried out and all planned permits issued to Maintenance team
at ST-30 and GT-33. ( including FVM11807)
GT-41, forced cooling completed, isolations carried out and At 08:08 Hrs, COP strainer removal permit
issued to GE. Another permit issued to GE to attend faulty WS 1AO-2 TC job.
CO2 plant-2, MEA filling carried out, isolations removed and taken in to the service.
GT-21 shut-down and de-synchronized at 22:33 hrs for HGPI, COP modification, Generator minor/magic
inspection. Forced cooling started.
On 22-03-2014
HRSG-33 main steam bypass vavle FVM-11807 stem found broken, it was replaced by MMD and permit
CLEAR at 03:02 Hrs.
ST-30 ,GT-33 & HRSG-33 isolations normalized, HRSG filling completed. ST-30 steam charging carried out
and starts command initiated at 05:27 hrs.
ST-30 synchronized with the grid at 05:55 hrs, GT-33 synch at 06:19 Hrs.
GT-41 offline water wash carried out for post HGPI performance test.
GT-21 forced cooling completed at 08:00 Hrs, generator degassing completed at 17:05 Hrs.
GT-21, HGPI & generator minor inspection permit issued to GE at 22:20 Hrs, COP modification permit
issued at 22:40 Hrs.
GT-41, Isolations carried out and permit issued for attending stage 9

bleed line flange leakage, then WI
valve stroking carried out by GE.
MED-239 tripped at 05:41 hrs due to primary condensate level HIHI during ST-0 startup as condensate
reject MOV got stuck in closed condition. EMD attended MOV. Operation checked and found ok. MED
started and distillate diverted to header at 09:02 hrs.
GT-41 started on liquid fuel on OPEN CYCLE mode for WI tuning, synch at 23:03 Hrs.
GT-41, Battery 15V DC Ground alarm, Battery charger # 1 AC Trouble and Battery charger # 2 AC
Trouble appeared. It was checked at local and found +ve ground fault alarm. I&C checked and asked to
synch, as it require shut down to attend. Notification 10011745 raised.
GT-41 Turbine compartment HAZ GAS LEVEL HIGH alarm appeared, (5C=5% and 5A=-1 %). Notification
10011744 raised. Haz gas online calibration carried out by I&C with RGC and alarm reset.

GT-41 load increased to 100 MW during WI tuning, WI pump cut off & GT run back due to H
2
O INJ
SUCTION PRESS HIGH LOCKOUT alarm. Suspected malfunction of WI regulator. I&C attended the WI
regulator which took 1:45 minuts, WI tuning resumed by GE.

MED-229 Leakage from GRP drain upstream of LCV-50302. Shutdown job.(Plugged temporarily)
GT-12 Inlet Heating Control Valve (IBH) 3% open showing at DCS and at local 5% open condition. Shutdown
job, Notification-10011587.
HRSG-33 main steam bypass MOV-11807 gland packing worn-out. Now leakage has increased and noise as
well.
GT-22 is having dropwise leak from one point, leak arrested by clamping. MMD will replace pipings in 2
weeks time (21.03.2014). On line water wash is being carried out.
BSDG sets, motor driven starting air compressor overhaul in progress (RTS tentatively 20
th
April).
HRSG-23 start up blow down MOV leak increased. (Shutdown job).
ST-10 generator casing test plug-H

leak. SAP-1100009848, Monitoring log sheet for LEL and pressure is
maintained regularly.
MPST-2 under shutdown for heat exchanger leaks. Nozzle cleaning not possible as isolation valve passing.
Heat exchanger sent outside for repair works. Expected back on 15.05.14
