Sir/Madam :

I am here to file for a formal complaint through Inquest Proceedings against our neighbor
Julo Culminas Jaugan, 27 years old, single a resident of Colon Extension, Brgy. Taclobo,
Dumaguete City Negros Oriental for the crime of Murder committed on June 28, 2014 at around
8:00 oclock in the morning at the aforesaid place. Said suspect is now detained at Dumaguete
Police Station after having been arrested.

To support my complaint, I have hereunto submitted the following documents

1. My affidavit
2. Affidavit of the witness Caroline Cruz
3. Affidavit of the arresting officer P03 German S. heperd
4. Photocopy of the Police Blotter
5. Photographs of the deceased
6. 3 kitchen knives (3) each measuring 24 inches.

AFFIDAVIT

I, Reynil Arcide Taleon, 32 years old, single, teacher, a resident of Colon Extension,
Brgy. Taclobo, Dumaguete City, Negros Oriental after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That I am the lawful brother of the victim Ken Taleon, also I am the complainant of
the case of Murder against our neighbor Julo Culminas Jaugan;

2. That on June 28, 2014, at around 8:30 in the morning, I was at Taclobo High School
Brgy. Motong, Dumaguete City, Negros Oriental to hold classes for my students when my
mother Rhea Taleon called and informed me that my brother was killed by our neighbor Julo
Culminas Jaugan;

3. That at around 9:00 upon arrival at our place in Colon Extension, Brgy. Taclobo,
Dumaguete City, I saw the lifeless body of my beloved brother on the ground next to our gate
outside our house;

4. That I am executing this affidavit to attest the foregoing facts in my own freewill and
volition to support the complaint against our neighbor Julo Culminas Jaugan for the crime of
Murder;

AFFIDAVIT

I, Caroline Cruz, 44 years old, married, housewife, a resident of Colon Extension, Brgy.
Taclobo, Dumaguete City, Negros Oriental after having been duly sworn to in accordance with
law, do hereby depose and say:

1. That I am the witness of the case of Murder against one Julo Culminas Jaugan;

2. That on June 28, 2014, at around 7:00 oclock in the morning, while I took water
from the well near the house of the victim, I heard a loud voice coming from the same house
saying in vernacular TABANG! NABUANG NAS JULO! PATYON KO NIYA! in English
HELP! JULO IS GOING CRAZY! HE WILL KILL ME!;

3. That I immediately looked where the sound came from and thereat I saw Julo C.
Jaugan, the suspect throwing a knife towards Ken Taleon, the victim hitting him at his back ang
goes through his chest. Julo C. Jaugan, not quite contented, threw two more knives hitting the
victim both times at the back which had made the latter fall to the ground. The three knives were
all embedded at the back of the victims body;

4. That after, Suspect Julo C. Jaugan went away with a blooded shirt, I shouted for help
and I immediately approached the victim and brought him to Silliman University Medical
Center;

5. That when we were on the way to the hospital, we met some Police Officers and
informed them of what happened;

6. That while at the Hospital, one of the Police Officers conducted investigation of the
killing that transpired in our barangay, Colon Extension, Dumaguete City;

7. That I am executing this affidavit to attest the foregoing facts in my own freewill and
volition to support the complaint against Julo Culminas Jaugan for the crime of Murder.

AFFIDAVIT OF ARREST

I, P03 German S. Heperd, 38 years old, married, bonafide member of the Philippine
National Police presently assigned with Dumaguete City Police Station and designated as Intel
Operative after having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am the arresting officer of the case of Murder against one Julo Culminas
Jaugan;

2. That on June 28, 2014 at around 9:30 in the morning, while I was at the Police
station, one Romulo Labrador, Brgy. Councilor of Brgy. Taclobo, Dumaguete City Negros
Oriental came and reported that there was a killing incident at Colon Extension, Brgy. Taclobo,
Dumaguete City Negros Oriental. The suspect was known as Julo and the victim, the latters
neighbor, as Ken;

3. That at around 11:00 oclock of the same date, the suspect Julo Culminas Jaugan
came to the station, accompanied by his father Roger Z. Jaugan, admitted that he killed Ken
Taleon and submitted himself for arrest. I advised them to wait at the Police Station;

4. That I together with SP04 Pete Bull and SP02 Dalma Xian proceeded immediately to
investigate the place where the killing incident happened and then eventually went to Silliman
University Medical Center where the victim was brought. We saw the victims lifeless body and
embedded on his back were three kitchen knives;

5. That while at the Hospital, I conducted investigation to the witness Caroline Cruz, she
disclosed to me that the victim Ken Taleon was attacked and killed by their neighbor Julo C.
Jaugan;

6. That around 1:30 in the afternoon of the same date, I returned back to our Police
Station and I placed Julo Culminas Taleon under arrest subsequently informed him of his
constitutional rights in a language known and understood to him as well as the nature and the
cause of his arrest;

7. That I am executing this affidavit to attest the foregoing facts in my own freewill and
volition to support the complaint against Julo Culminas Taleon for the crime of Murder;
