CHESTQUESTION 1
A 56-year-old man with known cirrhosis and portal
hypertension complains of progressive dyspnea on
exertion over the past 6 months. His symptoms have
progressed to the point that he is short of breath with
minimal exertion and occasionally at rest. He has no
other medical problems.
VOLUME 21, QUESTIQUESTION 1
His BP is 100/60 mm Hg, pulse rate is 88/min, and
respiratory rate is 14/min. Physical examination reveals
bilateral jugular venous distention to 5 cm above the
angle of the jaw. A grade III pansystolic murmur along
the left sternal border with inspiratory accentuation is
heard. Significant ascites and lower extremity edema
also is present.
