Sir:

This is to justify my outstanding performance for the school year 2013-
2014. I strongly believe that my performance is outstanding due to the
following reasons:

1. I have excellently achieved an outstanding performance in teaching
diverse learners and delivery of content knowledge in the
intermediate as manifested in the Summative test and Periodic test
Results of my pupils.
2. I was the trainer in the District and Division Science Camp with my
contestants garnering 1
st
place in the speech choir, and portfolio
making, second place in the environmental quiz and poster making
contest.
3. I was the trainer in the Science and Math quiz bee sponsored by the
DOST with my contestants garnering 1
st
place in the division and 8
th

place in the Northern Luzon Science quiz bee held at Benguet State
University.
4. I was the trainer in the Pisay CVC Science Quiz show held at
Bayombong Central School with my contestants garnering 3
rd
place.
5. I was one of the trainers in the Division NAMCYA competition held at
Solano East Central School and Jingle contest with our contestants
garnering 3
rd
place.
6. I was the trainer in the district and division English Read - A Thon
Competition with the following results:

8. I am one of the School Paper advisers and the trainer of the school
paper staff who represented the division in the regional press
conference held at Cauayan City last December 2013.

9. I was the English and Science trainer of the ACS Hamon sa
katalinuhan contestants garnering 1
st
place and 3
rd
place.


10. I served as a Cooperating Teacher of Janice Tullao a Practice
Teacher from the Nueva Vizcaya State University.
11. I served as lecturer and demonstration teacher during district
insets.
12. I served as proctor during School and District Competitions.
13. I organized my Homeroom PTA which assisted me in the
accomplishments of the following:
a. Changed the classroom chalkboard to white board.
b. Replaced the two destroyed galvanized roof of my classroom.
c. Replaced the wooden windows at the back of my room to
steel window.
d. Installed two new electric fans inside the classroom.
e. Repainted the frontage of Grade VI room.
f. Purchased of hot and cold water dispenser.

May I please inform your good office further that I have gained the
respect and admiration of my pupils and colleagues for I have
executed and played my role as stated in my duties and functions
professionally, sincerely and dedicatedly.
