KP3 - Installation
Integrity
Integrity means the risk of failure of
structure, plant, equipment or systems,
which could either cause or contribute to a
major accident and/or cause fatalities.
And.
It includes structures, plant, equipment or
systems that prevent or mitigate the effect
of a major accident and/or fatalities.

KP3 - Why Now ?

Ageing Installations
Industrys Commercially-Driven Changes
Manning changes
Maintenance/Inspection changes
Deficiencies identified in KP1
Specific Incidents e.g. Brent B

KP3 - Legal Basis of

Inspection
HASWA S2.2: Maintenance of plant, SFAIRP etc.
DCR Reg.8: DH shall ensure suitable arrangements for
the maintenance of installation integrity periodic
assessment remedial work
PFEER Reg.19: DH shall ensureall (PFEER) plantis
maintained in an efficient state, in efficient working order
and in good repair
PFEER Reg 13: DH shall take appropriate measuresto
protect persons from fire and explosion events.
PUWER Reg.5: Ditto for all work equipment

KP3 - Findings so far

9 Is the maintenance adequately structured

and resourced ?
9 Is the quality of maintenance acceptable ?
9 Does the performance of SCEs meet the
performance standard ?
9 Is supervisory and engineering support
adequate?

KP3 - HSE aims.

That KP3 is a joint and co-operative
venture with the duty holders
That we explain to responsible parties
what we are doing and why
That the workforce are fully on board, and
fully informed

KP3 - Inspection questions

What is the DH policy for PFP?
Are firewalls identified as SCEs?
Is the integrity of PFP included in
SMS?
