Thank you for your interest in Leadership Summit.

Leadership Summit dates are January 3 - 9, 2015. To attend (Credit or Audit) you must
complete and submit the following forms to the COBA Connections office by November 19, 2014,
at 5:00 p.m. Please complete application packet in ink.

COBA Connections will register you for the course December 2, 2014. As this is a January
short course, it will be calculated as part of your Spring semester hours and will be billed
accordingly. Tuition charges are covered by Block Tuition.
The program fee for Leadership Summit is $675. A non-refundable deposit of $250 is required
with your application. The deposit can be paid by check, credit card or cash and cannot be
applied to your student bill. The remaining balance of $425 will be charged to your account and
will reflect on your Spring semester bill. If you do not want the balance to be applied to your
student bill it must be paid in full by December 1, 2014. The program fee covers your lodging and
meals at Frontier Ranch.
Students are responsible for their transportation to and from Frontier Ranch. A bus option will
be provided. Round trip bus fare is $170. Seating on the bus is limited and will be assigned on a
first come first serve basis. A $60 non- refundable deposit is required with your application.
(Check, credit card or cash.) If you do not want the balance of $110 to be charged to your student
bill, it must be paid in full by December 1, 2014. The bus will leave Abilene on January 2, 2015,
and will return on January 9, 2015. (Times TBA)
IMPORTANT policies and deadlines to remember:
The deposits of $250 (program fee) and $60 (bus) are non-refundable and must
accompany your application.
The application deadline is November 19, 2014, at 5:00 p.m.
We are contractually obligated to Frontier Ranch to pay for your spot after December 1,
whether you show up or not. There are also other expenses incurred, before the event.
Therefore, the last day to receive the program fee reimbursement ($425) and/or bus
fee ($110) will be December 1, 2014.
No shows will be billed for the remaining program fee and bus fee if applicable.
The last day for tuition reimbursement is January 2, 2015. Proper procedures must be
followed with the Registrars Office to drop the course for tuition credit.

Release and
Indemnification
Agreement

WHEREAS, the undersigned is a student at Abilene Christian University (ACU) who has
voluntarily applied for acceptance to attend and participate in the Leadership Summit at the
Frontier Ranch located in Buena Vista, Colorado, January 3, 2015 through January 9, 2015
(hereinafter referred to as the SUMMIT); and
WHEREAS, the undersigned is aware of the risks associated with airline travel, highway
travel, and travel in the Buena Vista, Colorado area; and
WHEREAS, ACU cannot assure participants of SUMMIT that they will not be injured or
exposed to dangers and risks that may result in serious injury, loss, or harm; and

WHEREAS, the undersigned has carefully considered said risks and decided to assume same
in order to participate in the SUMMIT;
NOW, THEREFORE, the undersigned, for and in consideration of being allowed to participate
in the SUMMIT, agrees to and hereby does release, and hold harmless ACU & Frontier Ranch, its
officers, trustees, agents, and employees from and against any and all liability, loss, and damages,
including reasonable attorneys fees and court costs, which they or any of them may suffer as a
result of claims, demands, suits, loss suffered by the undersigned arising from or related to acts
or omissions of ACU, its officers, trustees, agents and employees in connection with travel and
transportation to and from the Frontier Ranch in Buena Vista, Colorado, and during the entire
time the undersigned is in Buena Vista, Colorado, or elsewhere as a part of the SUMMIT, and in
connection with any accident or injury that may occur during the activities associated with the
SUMMIT.
IN ADDITION, the undersigned agrees to comply with the ACU Student Guide and with the
rules and regulations of the SUMMIT trip to the Frontier Ranch in Buena Vista, Colorado, and its
duly constituted authorities, which may be in force during the undersigneds participation. The
undersigned agrees to act at all times during the SUMMIT prudently and to accept full
responsibility for his/her own actions and inactions while a participant in the SUMMIT Buena
Vista, Colorado trip.
This release and indemnification agreement shall apply to Abilene Christian University and
its representatives as described above and also to the duly constituted staff and representatives
of the SUMMIT.

Agreement
Leadership Summit in Colorado is an ACU sponsored academic course. All participants (for credit
or audit) will be subject to the ACU guidelines found in the ACU Student Guide. Frontier Ranch
policies also apply and these guidelines can be found in the Leadership Summit Workbook.
Failure to conform to ACU and Leadership Summit rules will be handled by the Leadership
Summit Director. If the offense warrants expulsion from the program, the student will be
responsible for all expenses incurred to return home.
The rules of dress and behavior are essentially the same as those on campus in Abilene as printed
in the Student Handbook (www.acu.edu/admissions/studentguide.html).
AGREEMENT:
I understand that the general guidelines of Abilene Christian University and the specific rules of
this agreement will be required for all participants of Leadership Summit.
I also understand and agree to abide by the following specific guidelines related to the Leadership
Summit:
No alcohol or illegal drugs are permitted on the Frontier Ranch campus, or on the bus
provided for transportation.
To remain on the Frontier Ranch campus, except when given permission by the
Leadership Summit Director or designee.
To attend all classes and all called meetings, including chapel and devotional times,
before and during the Leadership Summit experience.
I understand and agree that my failure to comply with any part of this agreement constitutes
grounds for dismissal from the program, and upon the recommendation of the Leadership
Summit Director, I may be sent home at my own expense.
