1-5:AC: Student omits entire section or >1 vital component of


history or PE section, OR adds component that was not
elicited in Hx taking or on PE.
NI: Student omits 1 vital component of Hx or PE section (this
also includes OSE, Vital Signs, and General PE).
M: Student omits non-vital pertinent positive or negative
features of Hx or PE, but nothing that clearly affects decision
making or Dx
Ex: Incorporates pertinent positive and negative information
in a logical format.
6. Assessment/Diff:
AC: Student omits entire Assessment, or fails to give a
differential with at least 1 plausible diagnosis.
NI: Student fails to provide 3 plausible differentials for
diagnosis, diagnosis should be one of the differentials, or fails
to defend first differential based on pts Hx/PE findings,
Student fails to include Somatic Dysfunction if applicable
M: Student is able to provide an appropriate primary
assessment with 3 plausible differentials and to defend the
primary diagnosis in diff, and includes Somatic Dysfunction
as a secondary assessment if applicable
Ex: Student meets all criteria for Meets and is able to provide
the most clinically relevant diagnoses in differential with
respect to most likely and most deadly, and defends all 3.
7. Plan:
AC: Student omits plan entirely, omits more than 2 parts of
the 5 part plan, or plan would result in death of the patient.
NI: Student omits 1-2 parts of 5 part plan, or OMT plan is
not appropriate for pt (note: if no OSE done, plan should be
to perfom OSE at next visit)
M: Student includes 4-5 parts of 5 part plan and OMT plan
is appropriate for pt.
Ex: Student plan is clinically appropriate for all parts of 5
part plan.

Comments (required for Exceeds or Area of Concern):

9. Time Management:
AC: Unable to complete a full section due to time
NI: Completes but only because some pertinent vital
information left out, or gets to Plan but unable to complete it
M: Completes presentation on time, with some pertinent, but
non-vital information left out, or added at the end
Ex: Completes presentation ahead of time with all pertinent
information included

8. Organization/Coherence:
AC: Presentation scattered with multiple items reported in
the wrong section (subjective in objective, plan in subjective)
and poor use of medical terminology resulting in limited to
no adherence to format
NI: Poor use of medical terminology and/or several items in
the wrong place, but basic format still present.
M: Correct following of format with few items in wrong
sections, majority of presentation uses correct medical
terminology
Ex: Excellent format, great use of medical terminology and
no items reported in the wrong sections
