Instructions

Requirements
To use the Care Plan Constructor the following are required:

A recent browser such as:


o FireFox 1.x
o

Internet Explorer 6.x

Safari 1.x

Opera 8.x

JavaScript must be enabled - Required for interactive functions including editing.

Cookies must be enabled - Required to save edits.

Navigation
Care plans can be found either by using the Diagnosis Index or the Symptoms, Diagnoses,
Procedures, Interventions & Clinical States search box.
Other pages can be found via navigation to the right of the "Care Plan Constructor" title.

Searching Symptoms, Diagnoses, Procedures, Interventions & Clinical States


The search box requires a minimum of two characters before it will act. Pausing briefly while
typing will result in a drop-down list of a maximum of ten possible matches. If more possible
matches exist a note directing you to narrow your search is also displayed. Selecting any of the
search results will cause a sublist of Diagnoses to display after a moment.

Saving, Opening, Deleting Care Plans Online


The toolbar found at the top of each care plan and the top of this page includes buttons labeled
Save, Open and Delete Saved Plans.
Save
Button becomes active after an edit has been made to a care plan. Saved plans remain
available as long as the browser is not closed and the session does not expire. These plans
are saved in edit mode and may still be changed.
Open
Button becomes active when saved plans are available. Clicking on a saved plan opens
that plan in edit mode.
Delete Saved Plans

Immediately deletes all saved plans. Plans will also be deleted when the browser is
closed.

Printing and Saving Care Plans Offline for Use in a Word Processor
All edits are included on the printable plan and checkboxes, select boxes, text boxes and Edit
buttons are removed. You will be asked to provide the name of the patient for whom the plan is
being created; this may be left blank. You will also be asked if Rationales should be included.
Pressing Continue will create the printable plan of care; Cancel closes the dialog box and
returns you to edit mode.
When no selections for a given checkbox-selectable section of the care plan are made (eg,
Nursing Interventions and Rationales) blank areas for handwritten notes will appear on the
printable care plan.
Printing unedited care plans will result in an error message. At least one selection or change must
be made.
Printable care plans may also be saved to your hard drive using your browser's save functions. If
it is not already, we recommend changing the ".php" extension to ".html" for easier use.

Making Selections
Checking and unchecking the checkboxes in edit mode selects and deselects items for the
printable care plan.
If more items are needed, use the Add...entries button. These additional items default to
Independent, but may be changed to Collaborative using the drop-down box. All added boxes
will appear on the printable care plan after the pre-defined ones in that section.

Making Edits
EditAn Edit button like the one to the right of this paragraph appears next to all Defining
Characteristics and Related Factors paragraphs and any selected items. Pressing this button
opens a box allowing edits. Your edits are not saved until pressing the Accept button, which also
closes the editable box. The Revert button in the toolbar at top is used to undo all changes made
to a care plan in edit mode without saving.
Within the edit box a modified version of BBCode is made available to you for limited style
edits and to separate the rationales from the core entry. The list below describes each of the
available codes. Learn more about BBCode.
Each tag is surrounded by square brackets ([ and ]) and end tags have a forward-slash (/) placed
after the left square bracket. Tagged text looks like this:
Every [b]good boy[/b] does fine.

and results in text displayed like this:

Every tag requires an end tag.

Creates a link to SOME URL.
Example:

list
A bulleted list container. Each item in the list is located between [list] and [/list] in item
tags.
item
The tag containing each entry of a bulleted list (list tag). A start and end tag are required.
Example:

rationale
A tag containing the entire Rationale for an entry. Rationales may be excluded from the
printable plan by indicating so after pressing the Create Printable Page button.
