IDENTITY CARD
EMSL ID card has been issued to you for identification purpose only. It should not be construed as an
authorization to the hospital to precede the treatment; however network hospitals may admit the person
as per their practice, pending authorization from us. The card should not be understood, considered,
accepted, presented or transacted as a credit card.
It is advised to check your previous policy details as registered with us and in case you have additional
information that you would like to register with us, please forward the copies to any of our branch office.

EMERGENCY HOSPITALIZATION AT NETWORK HOSPITAL

Cashless hospitalization facility is available only at Network hospital that enables the beneficiary to
obtain admission and take the necessary treatment at the selected hospitals subject to an Authority letter
(Pre-Authorization Letter) from EMSL.
The Authority letter or Pre-Authorization letter may be obtained against submission of duly completed
cashless request form. The certificate will authorize the hospital to provide the reported treatment for the
stated ailment, which shall be limited to the defined cost in the certificate.

PLANNED HOSPITALIZATION AT NETWORK HOSPITAL

In case of procedure like major/minor operations, delivery of child (where maternity benefits are
covered), Pre-Authorization Letter (PAL) is required to be obtained from EMSL at least 72 hrs before but
not before 30 days. The procedure to obtain is same as in the emergency hospitalizations.

Incase cost of the proposed treatment exceeds the authorized amount, new PAL is required to be
obtained to validate the additional amount.
If diagnoses or proposed treatment path changes for any reasons, it is important that a fresh PAL
is obtained on the basis of new medical inputs.

DENIAL OF CASHLESS FACILITY

(A) CONDITIONS WHERE A CASHLESS REQUEST MAY BE DENIED.
a) Where sufficient medical/ past insurance details are not available.
b) Where the reported symptoms/ available medical inputs are inadequate/ incomplete to
determine the liability of the insurer.
c) Where the intimation of claim/ hospitalization has not been given in time i.e. immediately in
emergencies or 72 hrs advance in planned hospitalizations.
d) Where any information had been concealed or misrepresented in the proposal form available on
record.
e) Where the reported ailment/ treatment is excluded under the policy.
f) In case personal information, policy and/or coverage description differ with records registered
with us.
g) Where the admission is of less than 24 hr durations unless specifically exempted in the policy.
h) Where the initial admission is for checkups, diagnosis including MRI, CT SCAN or other tests
unless specifically exempted in the policy.
(B) QUALIFICATION OF TREATMENT AGAINST DENIAL
Denial/ rejection of the cashless request does not mean denial of treatment or rejection of claims;
the insured retains the right to get the treatment within his capacity at his own cost and submit
his claims for consideration as re-imbursement case. EMSL shall re-consider all such claims on
the basis of additional medical and policy related inputs that may be provided by the insured at
the time of claim for reimbursement.

NON-CASHLESS/ NON-NETWORK CLAIMS

All claims where cashless facility has not been availed, pre-authorization request has been denied or
treatment has been availed at a non-network hospital are required to be settled directly with the hospital.
These claims may be submitted to EMSL for settlement on reimbursement basis.
It is important that all the relevant documents in original of the claims as described hereunder are
forwarded to EMSL. Incomplete documentation may delay the settlement of your claim.

GENERAL
It is important to know that:

All claims where cashless has not been requested to be informed to EMSL immediately as
described in the policy.
All claims that have not been intimated within the stipulated time limits under the policy may
not be considered for settlement and are subject to repudiation.
EMSL shall not be responsible for availability, quality and clinical outcomes of the treatment.
Insured is not bound to take the treatment only at the network hospitals, the network hospital list
is only for reference and extending the cashless facility.

Frequently Asked Questions (FAQs)

1) What is claim intimation?
Answer: Claim intimation is the notice of hospitalization/admission which is required to be sent to
any centre of E-Meditek in writing through letter/fax/email, 48hrs prior to planned hospitalization
or within 24hrs in case of emergency hospitalization. The claim intimation should include all the
relevant details like Name of Patient, age, gender, contact details, diagnosis, name-address-contact
details of the Nursing home/hospital, date of admission along with a copy of Admission Note of the
treating doctor.
2) Where do I submit the document?
Answer: You can submit the documents to the nearest E-Meditek branch (branch details has been
provided in benefit guide book) OR You can send the documents to E-Meditek head office at the
following address, E-Meditek (TPA) Services Ltd. Plot No.- 577, Udyog Vihar, Phase - 5 Gurgaon,
Haryana - 122016
3) What I have to do for availing cashless hospitalization facility?
Answer: In case of planned hospitalization, E-Meditek should be informed 48 hrs in advance,
through the duly filled-in/ Pre Authorization form (available at E-Meditek website/ at the TPA desk
of the concerned hospital). Ensure that the Pre Authorization Form is signed by the member and the
treating doctor. Your claim would be assessed as per terms and conditions of the policy by the
insurer and a letter of authorization will be issued to the hospital authorizing treatment.
In case of emergency hospitalization/admissions, the concerned hospital is required to send the
request for Authorization for cashless hospitalization to E-Meditek with in 24 hrs of
hospitalization/admission. Once E-Meditek authorizes the concerned hospital, the cashless treatment
through an Authority letter, you are not required to pay the hospital up to the amount which has
been sanctioned by the said authority letter. E-Meditek will pay your hospital bills up to the amount
authorized in the Authority Letter only.
4) What is the time limit for claim submission for reimbursement in case of hospitalization?
Answer: All the claim documents should be submitted within 30 days from the date of discharge
from the hospital to any nearest E-Meditek Branch Office or Head Office.
5) Where can I avail cashless hospitalization facility?
Answer: You can avail the cashless hospitalization facility in the Hospital mentioned in the
guidebook under the head "List of empanelled hospitals for cashless hospitalization under TPA". The
updated list of such empanelled hospitals may also be seen at the TPAs website
(www.emeditek.com)
6) What are the documents required to be submitted for claiming reimbursement for hospitalization?
Answer: Kindly submit the following documents for processing your claim.
1. Duly filled-in claim form
2. Original discharge card
3. Original hospital bill
4. Original hospital bill payment receipt, and all the prescriptions.
5. Original investigation reports
6. Medicine Bills
7. ECS form duly filled-in along with the cancelled cheque.

8) What do you mean by Pre-Hospitalization & Post hospitalization?
Answer: The Pre hospitalization period is 30 days prior to Hospitalization and Post Hospitalization
period is up to maximum of 60 days from the date of discharge from the Hospital.
9) What is the procedure for claiming reimbursement for Pre hospitalization as well as Post
hospitalization period?
Answer: The expenditure incurred during 30 days prior to Hospitalization (Pre hospitalization) and
60 days from the date of discharge from the Hospital (Post Hospitalization), for the treatment of a
particular disease for which Hospitalization takes place, is considered for reimbursement. This along
with the claim form and prescriptions, medicine bills, investigation reports, investigation advices
should be submitted.
10) My Spouse cannot sign on claim form can she give left hand thumb impression on Claim form?
Answer: Yes, she/he can fill the claim form with left hand thumb impression on Claim form and
submit the documents.
11) Are only diagnostic test covered?
Answer: Diagnostic tests which are not consistent with line of treatment are not payable at all.
However, Charges for diagnostic test consistent with or incidental to the diagnosis and IPD
treatment of the positive existence or presence of any ailment, sickness or injury are payable.
12) What do I need to do at the time of discharge in case of a cashless facility?
Answer: You need to sign your bills, fill up a claim form and sign the same, leave all your
investigation/diagnostic reports and X-ray/ultrasound films etc. with the Hospital. The hospital
authorities will send the documents to us for assessment and payment.
13) What are the details on ECS Payments?
Answer: Electronic clearance system is process where the amount is directly transferred to
claimant's account. Claimant needs to fill in the ECS form available on our website
(www.emeditek.com) or avail it from our branches. Once duly filled they can send the document to
the branch or Head office along with a cancelled cheque.
14) What is the process of availing cashless facility in the hospital?
Answer: In case of a planned hospitalization, client needs to inform E-Meditek 28 Hrs in advance by
faxing us the pre-authorization form (form on Website) signed by the treating doctor. Your claim
would be assessed in the light of the policy issued to you by your insurance company and a letter of
authorization will be issued to the hospital authorizing the patient's treatment. In case of an
emergency the hospital will fax the pre-authorization letter. Once the Authority letter is sent to the
hospital from E-Meditek, you need not pay to the hospital. E-Meditek will pay your hospital bills up
to the amount authorized in the Authority letter.
15) What are the formalities to be followed after discharge of patients in case of reimbursement facility?
Answer: You need to follow the below mentioned process.
Completely fill the claim form with address and contact number of the claimant
Discharge Summary of the hospital in original mentioning the following:

o Date of Admission
o Date of Discharge
o Final Diagnosis
o Disease since when------with original first consultation paper
o Treatment provided
o Condition on discharge
Final Hospital Bill
The complete breakup of the final hospital bill (breakup of medicines given, laboratory tests)
The receipt of payment in lieu of the final hospital bill
All the investigation reports (e.g. blood test reports, X ray films and reports, MRI films and
report)---whichever applicable.
All the bills/receipts for which claimant is claiming.
All the prescriptions, medicine bills and the investigations report
All past policy copies if any

Documents should be submitted to E-Meditek, within 15 days of discharge from the hospital. All the
above documents have to be submitted in original and have to be couriered to the nearest E-Meditek
office.
16) What are our rights and duties as a policy holder to the hospitalization?
Answer:

You have a right to be treated in any duly registered hospital of your choice as per Policy terms &
conditions
You have a right to know the status of your claim and if your claim has been found admissible
you have a right to be paid
You have a right to represent to us or to the insurance company in case you disagree on any
matter

Inform E-Meditek in case of an admission (Intimate your claim) as per Policy terms & conditions
Provide complete and correct information as required by your treating doctor who would in turn
provide the same to E-Meditek
Send all relevant documents to EMSL. Please quote your policy number/card number.
17) On hospitalization, what is the time frame for intimation to the TPA or the Insurance Company?
Answer: You are required to intimate the TPA or your insurer within 24 hours of hospitalization.
18) What is the time frame for submitting your reimbursement claim after getting discharge from the
hospital?
Answer: You are required to submit your reimbursement claims within 15 days from the date of
discharge.
19) In case we get treated in a non-network hospital, where should we lodge our claims?
Answer: You can lodge your claims at our office at Plot No. 577 Udyog Vihar Phase V, 122016. Send
your intimation to intimation@emeditek.com or call us at 1800 102 3242 on a 24x7 basis. If your

claim is found admissible, it shall be paid within 15 days of receipt at our end.
20) Can medical costs be reimbursed from day one of the cover?
Answer: Typically there is a waiting period of 30 days, within which no claims by the insured are
entertained by the insurer. This waiting period may vary from one insurance company to insurance
company. We strongly recommend you to read your policy document carefully and clarify the
matter with your insurance agent.
21) If we have a health insurance policy in Mumbai, can we make a claim in case of Transfer to Delhi?
Answer: Yes you can, all health insurance policies are valid across the country.
22) Is there a minimum time limit for stay within the hospital under the health insurance plan?
Answer: Typically, the insured can make a claim if his/her hospitalization is more than 24 hours.
However, for certain treatments, such as dialysis, chemotherapy, eye surgery, etc, the stay could be
less than 24 hours.
23) How can I view my profile detail on E-Meditek Web site?
Answer: Login to E-Mediteks website (www.emeditek.com), click on the User login option at
right hand side corner of the website, to proceed enter your user name and password in the given
option.
24) How to download the E-Card from web site?
Answer:
Login to www.emeditek.com and click User login

25) Where and how to check network provider list?
Answer: Please visit www.emeditek.com, click on network provider if you are a corporate policy

26) How to enroll dependents name under Mediclaim insurance policy?
Answer: To enroll the dependent's name under the Mediclaim policy
You must directly contact the insurance company
The insurance company will forward the endorsement letter (for adding dependents) to EMeditek
Then E-Meditek will add their dependents name(s) under his policy.
27) How to correct mistake in TPA Card?
Answer: If there is any mistake in TPA card like: Name, Gender, Age, Photo etc, you can send an email to customercare@emeditek.com for correction and also you can call us on 18001023242,
informing the customer care department to make the requisite changes.
28) How to correct mistake in policy?
Answer: If there is any mistake in policy like name/age/gender/address etc, you need to contact the
insurance company; they will forward us the endorsement letter for the necessary correction. After

receiving the endorsement letter from the insurance company, E-Meditek will make corrections in the
requisite field.
29) Can we claim medical expenses incurred before and after the hospitalization?
Answer: Yes, under the Mediclaim policy, medical expenses from the date of admission before 30
days will be incurred as a pre-hospitalization and from the date of discharge after 60 days will be
incurred as a post-hospitalization.
30) What are pre-existing diseases?
Answer: Before taking Mediclaim policy if there are any existing diseases before taking the policy,
these diseases are classified as pre-existing diseases.
31) What are the Non-Mediclaim expenses?
Answer: The non-Mediclaim expenses are mentioned as below:

32) In case a person has a corporate policy but he has resigned from the corporate last week, can he take
the treatment benefit from this policy?
Answer: After tendering his/her resignation, client cannot continue this policy - it will lapse once we
receive the e-mail from his/her HR dept regarding the employee's resignation from the company.
