POST DOCTORAL RESIDENCY AGREEMENT

NORTHSHORE PSYCHOLOGICAL ASSOCIATES (NPA)

A.

Whereas Resident is in need of qualified post doctoral


supervised hours leading to licensure as a psychologist in
Pennsylvania and NPA is interested in providing such
supervision, Resident and NPA agree to the following
arrangement.

B.

The residency period will consist of 2000 hours provided


over the course of a calendar year beginning _____________
and ending ________________.

C.

Supervision will be provided by Primary and Secondary


listed above, who are licensed in the Commonwealth of
Pennsylvania as psychologists and have no current
disciplinary complaints against them by the State Board
of Psychology (SBOP). Ancillary supervision may be
provided throughout the year by other professionals
within the practice as appropriate.

D.

Supervision will be provided consistent with the standards


for supervisors set by the SBOP (See attachment A) which
covers in detail 19 elements related to the supervision of
psychological residents.

E.

Supervision will consist of a minimum of 2 hours of face to


face individual supervision with either Primary or
Secondary.

F.

Additional learning objectives, specific to the Resident,


will be developed during the initial month of residency
and presented for inclusion in the Residents overall
learning plan.

G.

Professional conduct of both the resident and NPA staff will be governed by
the APA code of Ethics, standards set by the SBOP and NPAs office policies.

H.

Evaluation of Resident will be done consistent with SBOP guidelines utilizing


the Psychology Trainee Competency Assessment Form (See attachment B).

I.

Performance problems or conflicts related to the Residents relationship with


supervisors, other staff or training experiences will be handled through the
Conflict Resolution Policy with modifications currently in effect for the Erie
Psychology Consortium (See Attachment C).

J.

Resident will be a per diem employee of NPA at a rate of $______ per hour
and subject to all office policies. No benefits are associated with this position.
