CEMENT SELECTION

Preparation Tip:

Adhesive preparations, also known as non- Taper greater than 8°

retentive preparations, include tapers in
excess of 8°. In these situations, or in “short
prep” situations (ie, less than 4 mm in height),
adhesive resin cements are recommended to
achieve sufficient retention.
Retentive preparations feature a taper
between 4° and 8°. In these situations
restorations made of high-strength ceramics
Taper between 4° and 8°
(eg, lithium-disilicate) or restorations with a
strengthened core (eg, metal, zirconia) may
be adhesively cemented or conventionally
cemented. While adhesive cementation
always provides the strongest “bond” and
the best marginal seal, the luting forces of a
conventional cement are sufficient for both
retention and support of the restoration.

Metal and Metal-Based Restorations

Many clinical situations (eg, cast endodontic posts, patients with parafunctional habits) are
indicated for a restoration with a metal substructure. These restorations feature retentive
preparation designs and can be seated with adhesive or conventional cements according to
the following protocol:

Glass-Ceramic Restorations
(Leucite-Reinforced, Fluorapatite, Feldspathic)
Bonded metal-free restorations fabricated from leucite, fluorapatite, or feldspathic glass-
ceramic materials provide patients with lifelike esthetics and vitality. These restorations
obtain additional support from the adhesive bond to the underlying non-retentive prepara-
tion and benefit from an adhesive cement.

High-Strength Ceramic Restorations

(Lithium Disilicate, Zirconia, Alumina)
High-strength ceramics have further expanded the clinician’s ability to provide durable,
esthetic restorations. Depending on the ceramic and clinical situation, the restoration may
require adhesive bonding or conventional cementation. Whether these materials are used as
a high-strength core and then overlayed with another ceramic (eg, e.max ZirCAD), or if they
are milled to full contour (eg, e.max CAD LT), the following cement choices are available.

Indirect Composite Restorations

(Reinforced Composite)
Similar to a glass-ceramic restoration, an indirect composite restoration is strengthened by
the adhesive cement used for its retention. Cement selection is, thus, largely determined
by the extent of the adhesive preparation. When the preparation is confined to enamel, the
etch & rinse technique is optimal; the self-etch approach is indicated for dentin bonding as
in the case with composite posts.
