Internal DBHIDS Transformation Champion

Awards
In 2005, the City of Philadelphia made the decision to transform its behavioral health and
intellectual disability services systems to improve the health status of Philadelphians in need of
behavioral health and intellectual disability services. Since then, DBHIDS has been at the
forefront of international efforts to transform systems of care with an emphasis on recovery and
resilience for behavioral health services, and self- determination for individuals with intellectual
disabilities.
The transformative changes within our Philadelphia system required the creation of a community
wide strategy, which included the shared visions and tireless efforts of leaders, providers,
community stakeholders, people in recovery, individuals with intellectual disabilities, DBHIDS
staff, and more.
To commemorate the ten year anniversary of system transformation in Philadelphia, coined the
Transformation Decade, DBHIDS has hosted a series of events since February to highlight
accomplishments, celebrate those who have played integral roles in the transformation of our
system, and look toward the future. The 10 month long celebration of our Transformation
Decade will conclude in November 2015 with a final celebration for our entire system.
From its inception to the present, internal DBHIDS staff have been integral in exemplifying the
DBHIDS transformation principles and supporting recovery, resilience, and self-determination
for individuals, families, and communities. Help us continue to celebrate our Transformation
Decade by acknowledging and celebrating our Transformation Champions within DBHIDS.
I invite you to nominate DBHIDS staff for a Transformation Champion Award by submitting the
official Transformation Champion Award nomination form electronically to
alma.taylor@phila.gov by the deadline date of October 1, 2015. Completed Transformation
Champion Award nomination forms can also be submitted via Transformation Champion
nomination boxes, located at each of the five DBHIDS locations.
Transformation Champions will be identified in each of the following categories: Self
Determination & Brighter Futures; Community Mental Health & Wellness; Children & Youth;
Cross Systems Collaborations; Align, Coordinate, & Integrate: Innovations & Improvements in
Service Delivery; Addiction Services & Recovery Advocacy; Peer Culture & Family Inclusion;
and Unsung Heroes. Transformation Champion Awards will be presented to internal DBHIDS
staff at the Transformation Decade Award ceremony in October 2015.
I strongly encourage you to nominate your colleagues; Transformation Champion Awards are a
wonderful way to celebrate and recognize the positive impact your contributions have made on
the lives of Philadelphia citizens.

ADDICTION SERVICES & RECOVERY ADVOCACY : Nominee has shown dedication to
transforming efforts in the areas of substance use prevention, intervention, treatment, and
recovery/resilience advocacy.
PEER CULTURE & FAMILY INCLUSION: Nominee has exhibited influence as a source of
peer support/leadership and/or family member advocacy within the areas of behavioral health
and intellectual disAbility services.
UNSUNG HERO: Nominee has exhibited dedication to transforming our behavioral health
system, embodies the ideas of recovery, resilience, and self-determination, and was instrumental
in implementing change in areas of DBHIDS not otherwise included in the above categories.
