RELEASE AND WAIVER OF LIABILITY

THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS!
This Release and Waiver of Liability (the Release), executed this
day of 2015 by
___________________________________ (Visitor), in favor of Kenneth W. Rendell, Individually, Kenneth W. Rendell, Inc.,
and the Museum of World War II, Inc., their Directors, Offices, employees, volunteers and agents (collectively the Museum).
The visitor desires to visit the Museum of World War II and acknowledges and understands that the artifacts are primarily of
World War II vintage and are instruments of war, and that the layout of the premises and said artifacts within the said premises are
distributed in such a manner to be closely observed by the visitor and presented in such a manner that they may not expose the
visitor to contact and/or injury.
The visitor hereby freely, voluntarily and without duress executes this Release under the following terms:
RELEASE AND WAIVER. In consideration of being granted access to the Museum. Visitor does hereby release and forever
discharge and hold harmless the Museum and its successors and assigns from any and all liability, claims and demands of
whatever kind or nature, either in law or in equity, which arise or may hereafter arise from visitors visit to the Museum.
The visitor understands that this Release discharges the Museum from any liability or claims that the visitor may have against the
Museum, with respect to any bodily injury, illness, death or property damage that may result from visitors activities within the
Museum, or on the Museum premises, whether caused by the negligence of the Museum or its officers, directors, employees,
agents or otherwise. Visitor hereby expressly and specifically assumes the risk of any injury or harm in visiting the Museum, and
releases the Museum from all liability for injury, illness, death or property damage resulting from such visit.
Visitor expressly agrees that this Release is intended to be as broad and inclusive as permitted by the laws of the Commonwealth
of Massachusetts, and that this Release shall be governed by and interpreted in accordance with the laws of the Commonwealth of
Massachusetts. Visitor agrees that in the event that any clause or provision of this Release shall be held to be invalid by any Court
of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this
Release, which shall continue to be enforceable.
In witness whereof, Visitor has executed this Release as of the date and year first above written.
