HEALTH CHECK PROCESS

The following information has been put together to help you while you are undergoing the health check
at Columbia Asia Hospital, Whitefield. In case of any further queries, please contact the health check
coordinator who you will meet once you are here.
1. Registration:
a. This is the first step in the whole process. Registration involves creation of a unique
hospital identification number and the electronic medical record of the patient. Please
let customer care know if you already have a registration number in the hospital.
b. You will be handed over a registration form, please ensure the details that you fill in are
correct. In case changes are required subsequently, a separate process will be followed.
c. A photo id card will be issued, please ensure that you collect the card before you leave
the hospital
d. Please double check the contact numbers you provide during registration, these will be
used in case of an emergency
2. Sample collection:
a. Blood and urine samples will be collected at the phlebotomy area in Nurse Station A
b. Please ensure you follow health check guidelines on fasting etc.
c. Please ensure customers taking above 40 package must carry there stool sample
3. Breakfast:
a. You will be provided a complimentary breakfast by the health check coordinator.
b. Breakfast is served on the Ground Floor in the health -check area
c. Please make a note of the time that you finish breakfast, the post prandial samples
should be given about 2 hrs after your breakfast.

4. The remaining tests will be conducted after your breakfast


5. Consultation with the Ophthalmologist for Eye will be fixed after breakfast once all samples are
collated. The OPD consultation will be fixed for the next following day (only on working days).

General Guidelines
1. Abstain for consuming alcohol at least 12 hours prior to the health check
2. You will require to be fasting for 10- 12 hours. Water can be consumed but not hot
or cold beverages

3. Kindly carry your medical records that are 1-3 years old and medical accessories like
spectacles, hearing aids if you have any when you come for health check

4. Let us know if you are pregnant or suspecting pregnancy. In such cases x ray will not be
done

5. Please do not apply cream, oil or powder on your chest when you come for the health
check

6. Wear loose fitting clothes and avoid wearing jewellery like necklace, chain etc.
7. Plan to arrive at the hospital at 7:45 am on the day of the health check
For women

1. You are advised not to undergo the health check during ongoing menstrual period. The
result of the urine examination and the PAP smear gets altered during this period

2. Radiological investigations (X-rays) are not advised in the early stage of pregnancy.
3. Mammography is usually not recommended for women less than 40 years of age.
Special Precautions

1. Let us know in advance if you are a diabetic/ cardiac patient


2. Kindly carry your prescription/ medication along with you if you are taking any
3. If you are a diabetic on medication, please take the medication as per your normal
schedule

4. If you are a hypertensive on medication please take the schedules medication at 6:30
am on the

day the check up
