EE/IM4040: Engineer & Society Tutorial 5, March 26-27 March, 2015 Group 1.

Members of P5 are to divide the questions fairly. Answer briefly & concisely, summarising
main points only. All students are to bring Safety lecture materials for revision during
tutorials.
Q 1. A fresh EEE engineer, Mr Y (Y) works for a company dealing with the storage of
flammable gases in a Refinery in Singapore. He was in charge of the project below:
A contractor, Tan Bo Chaps task was to design and install power lines in a cable tray above
some holding tanks of flammable gases in the Refinery. (See CaseStudy pix. Use for
presentation.) As Ys company does not have the resources, they have a term in the contract
with contractor Tan Bo Chap stating that the contractor is fully responsible for all safety &
health duties for the project.
A cherry picker (mechanical elevated platform) was used. 2 workers were involved in the
work. As one worker could not reach the roof, he took off his lanyard from the basket of the
cherry picker & climbed on top of the roof to do the job with a cutting torch. An explosion
occurred. The worker slipped & fell to his death. The other worker wearing the lanyard
suffered 20% burns.
In answering questions below, state any assumptions made & quote the relevant sections
of the Workplace, Safety & Health Act, Cap 354A of Singapore Statutes (referred to as
WSHA). Draw diagrams showing the persons involved, the injured parties & their capacity.
(a) (i) What are Tan Bo Chaps duties and Ys duties in relation to 2 workers above? (See
Interpretation section & duties of WSHA).
(ii) Did Ys company contravene any duty or duties under the WSHA? State assumptions
made.
(b) Look at the work process of installing power lines in a cable tray above some holding
tanks in the Refinery. Break down the process eg bringing tools to cherry-picker & unloading,
bringing in the cherry-picker, cutting steel sections and using electric drills to secure tray
sections to roof, etc. Choose 2 significant hazards from this case study. Perform a risk
assessment & suggest control measures if the risks are high. Use the Risk Assessment Form
in the Standard Operating Procedures (SOP)for Risk Management in NTUs OHS website in
Reference (1) (c) below.
Q.2 Explain Reasons Cheese Model.
Is it applicable to the accident in the casestudy? Why or why not?
