TIME: one hour from 08:30 till 09:30 am
Answer all questions. The space provided for answer should suffice. Answers need
not be too detailed. However, for more space, use the back of the sheet. But refer to
this by writing PTO. MC question may accept multiple answers.
1. The Following table gives you some root causes of accidents or incident; some of them are (1) unsafe act,
(2) some are unsafe procedures and others are (3) management deficiencies. In the free column write
which root cause is related to one or more of the previous categories (1 to 3)? Under definition put your
answer in numbers (e.g. 1, and/or 2, and/or 3) a root cause may belong one or more of the three
definitions.

Definition:1 and/or 2/3

The following concepts are important for any good risk observer:
) adopting synergy
) being proactive
) believing in continuous improvement
) being aware of the influence of mental models or perceptions
) having general and diversified knowledge
) adopting teamwork style
) giving individual empowerment (independence in taking decision)
Check only the true answers of the following
) Something that could kill a person must be of very low probability.
) Always the severe accidents have high probability in order to be considered as a high risk.
) Risk is a way of introducing uncertainty in the decision making process.
) A high risk activity is quite likely to cause a lot of harm.
) If something is very risky then it must also be very difficult to do.
) Everyday desirable activities like playing sport are not risky at all.
) Risk is mostly to do with industry and accidents at work.
) Earthquake could be considered as an accident, an incident, or even as a near miss.
) Near miss is the easiest event to be recorded.
) Using seat belts controls risk by reducing the probability of having severe consequences.
) Using seat belts controls risk by reducing the severity of consequences.

Which of the following is not considered a source of undesired events?
( ) Fireworks stored in a warehouse
( ) Smoking

10. You are a group of students making a model for a piece of equipment. Define the various undesired
events that may happen to you (the group) from the time you receive the assignment till the time you
submit it. Follow the 5-step risk approach. Vague answers are not appreciated.
Note: give a title for un-titled steps 1, 2, 4 & 5 (follow the example of 3a & 3b)
Step1: System Definition
System Objective: implementing the task (good equipment design, building a good report, ) in one month from now
We are a group of four students (all male or all female or mixed)
Work place: at the faculty during study hours. At home (individual work). At a team member flat (group work). One of team members has a
dog at home (a pit bull, always left loose in the house)
Team member homes: close or distanced
Ways of communication: direct during meetings; mobile, internet, phones, written messages (while apart)
Equipment & assets: laptops, printers (of which only one is colored, where?), one car, mobile phones, others required. This is besides the home
and home contents
Division of work: may not be clear at the start. Will take some time to define based on the evolution of the team
Cultures: all compatible, team members are willing to cooperate and spend the effort. Are some of them do not synergize efforts with the
others?
Environment Internal, external: December 2009 through January 2010 (winter time)
Step 2 Hazard Identification
Hazards related to persons, harmony, characters, willingness to spend the effort, to cooperate, and to add value to the team, awareness of the
importance of achievement of objectives
Assets and equipment are good, functioning properly, well maintained so that there will be no delays. Everything is planned so that interactions
between all parts of the system do not impede operations and cause harms to systems objective. Possible hazards are
Delays: lost time before full coordination, lost time due to lack of synergy between persons; delays related to other persons outside of the group
Members of the group do not have the required skills
Disputes and misunderstanding between the members of the group and/or other persons
Un-availability of equipment or tools at the proper time
Problems related to the loose animal, pit bull, which is a very aggressive type of dog.
Problems related to the car, communications, distance between work areas
Loss of other resources
Hazards due to data, viruses, lack of monetary resources
Possible gender related hazards, for mixed groups
Step 3a: Undesired events that could result from Step 2
Damage to assets or equipment
Delays, time or financial
Loss of data
Social conflicts
Step 3b: Few consequences
Any health hazards: stress, fatigue, hurt by the animal
Getting poor grades
Loss of image of the group and loss of trust between the members
Possibility of not achieving the objective
Step 4: Risk Evaluation
Undesired Event
1. Any health hazards: stress, fatigue, hurt by the animal
2. Getting poor grades
3. Loss of image of the group and loss of trust between the members
4. Possibility of not achieving the objective. Depends on the quality of the system.

Risk Control
Keep dog away, better planning diminishes stress &
social rapports between groups
Good planning and preparation and division of work
between members. Good selection of team members
prevents most of these problems.
