Resumen
Las enfermedades tiroideas pueden ser diagnosticadas, en la mayora de los casos, por
la clnica. Sin embargo, existen situaciones que requieren de las pruebas de funcin
tiroidea con el fin de valorar correctamente al paciente en las etapas iniciales de la
disfuncin de esta glndula, con el fin de establecer el tratamiento adecuado de forma
individual y el seguimiento de la evolucin de la enfermedad. En esta revisin hacemos
referencia a las pruebas empleadas para valorar el estado de la funcin tiroidea y se
sealan sus ventajas, limitaciones y tendencias actuales. La determinacin de TSH por
procederes de segunda y tercera generaciones permite establecer el diagnstico del
hipertiroidismo e hipotiroidismo primarios, an en las formas subclnicas de ambos, al
mostrar inhibicin o hipersecrecin, respectivamente, as como la dosis adecuada de llevotiroxina y orientar a una causa hipofisaria de esta disfuncin. La determinacin de
T4 es de gran valor para conocer la intensidad de la disfuncin tiroidea y para valorar
la eficacia del tratamiento en las primeras semanas de iniciado. La determinacin de T3
tiene su principal indicacin ante la sospecha de la presencia del llamado
hipertiroidismo por T3. La determinacin de T4 y T3 totales da lugar a una
interpretacin incorrecta en cuanto a las situaciones que modifican la globulina
transportadora de tiroxina (TBG). La tendencia actual es al empleo de TSHs como
prueba inicial para el diagnstico de la disfuncin tiroidea, la cual se debe indicar
teniendo siempre en cuenta la clnica y, de ser necesario, asociarla a la determinacin
de T4 o T3 libres.

Las enfermedades del tiroides constituyen una de las endocrinopatas ms frecuentes,
si no la ms comn. Aunque en un porcentaje elevado el diagnstico puede
establecerse por la clnica, en aquellas que repercuten ms seriamente sobre la calidad
de vida del ser humano esto no siempre resulta fcil, sobre todo en las etapas iniciales,
lo que obliga a la realizacin de numerosas pruebas de laboratorio, las cuales son
tambin necesarias para su valoracin postratamiento. Por eso, resulta evidente la
necesidad de contar con procederes diagnsticos que permitan confirmar la impresin
clnica y mantener un seguimiento correcto con pruebas ms sensibles, lo cual
posibilitar reducir el nmero de estas, as como obtener los resultados en el menor
tiempo posible por extraccin de la muestra de sangre. A las pruebas pioneras, como
el metabolismo basal, el PBI y la captacin de yodo radiactivo, se han ido aadiendo, a
partir de la dcada del 70, las determinaciones de la fraccin total de las hormonas
tiroideas T4t y T3t por RIA y el estimado indirecto de la fraccin libre de T4-T3,
mediante el empleo de otros procederes (relacin T4t o T3t/CT3 y TBG) as como la
determinacin de TSH por RIA, lo cual constituy un gran avance en este campo. En la
bsqueda de procederes con ms posibilidades diagnsticas y en los cuales uno de
ellos fue capaz de sustituir a los otros en la mayora de las situaciones clnicas, lo que
hizo ms econmico el estudio de los pacientes, se lleg a la determinacin directa de
la fraccin libre de T4l y T3l y a la de TSH por mtodos sensibles (TSH) de segunda
Los valores sricos de T4t estn influenciados por los niveles circulantes y la afinidad
de las protenas transportadoras, principalmente la globulina transportadora de tiroxina
(TBG), lo cual interfiere en la valoracin correcta de los resultados en diversas
circunstancias, especialmente por la existencia de niveles elevados de estrgenos
circulantes (gestacin, ingestin de anovulatorios que contienen estrgenos y en la
terapia hormonal de reemplazo) as como por la ingestin de 5-fluoracilo y clofibrato
que determinan niveles elevados, y de andrgenos y danazol, que producen valores
bajos.11,12
Debemos tener presente que la ingestin de salicilatos, diazepan, fenilbutazona,
carbamazepina, fencofenaco y difenilhidantona, al competir con la protena
transportadora por su unin a T4, pueden determinar valores bajos.13 Por eso, al
interpretar los resultados hay que valorar la posible ingestin de dichos medicamentos
de forma mantenida.
Para obviar esta situacin que afecta la determinacin de T4t, se recomienda la
determinacin directa de la T4 libre (T4l) o de forma indirecta, mediante la
determinacin del ndice de T4 libre (IT4l = T4t/CT3) o el clculo de la relacin
T4t/TBG, los cuales pueden ser tiles en casos de variaciones no intensas de las
protenas transportadoras, pero implican la realizacin de otras pruebas, lo que
encarece la determinacin. Es necesario tener presente que de ninguna forma pueden
sustituir a la determinacin directa (T4l) de la hormona cuando existen anormalidades
intensas de las protenas transportadoras, como en la hipertiroxinemia
disalbuminmica familiar,14 o por modificacin de la TBPA (prealbmina o transtiretin)
que aumenta su afinidad.15
La concentracin de T4, tanto en la forma de T4l como en la T4t, estar disminuida en
el hipotiroidismo no solo primario sino tambin hipofisario e hipotalmico, y aumentada
en la tirotoxicosis asociada a valores elevados o disminuidos de TSHs,
respectivamente.
La determinacin de T4, tanto combinada como libre, en pacientes con TSHs alterada,
permite una mejor valoracin de la intensidad de la enfermedad tiroidea que motiv
esta investigacin. Tambin permite valorar la utilidad del tratamiento del
hipotiroidismo e hipertiroidismo primario en las primeras semanas de su aplicacin.
En pacientes con enfermedades severas no tiroideas es posible detectar valores bajos
de T4l o del IT4l con restauracin de la normalidad cuando el paciente se recupera.1620 Se ha reportado discordancia entre la determinacin de IT4l (valores bajos) y la
determinacin de T4l (valores normales) en pacientes con enfermedades severas no
tiroideas,6 atribuidas a la disminucin de la concentracin de TBG o a cambios en su
La determinacin de T3t y del ndice de T3l (IT3l) presentan las mismas dificultades
sealadas para la determinacin de T4t e IT4l, y l mismo proceder empleado para
calcular el IT4l puede ser aplicado para calcular el IT3l.
La concentracin srica de T3 se encuentra aumentada en el hipertiroidismo y
disminuida en el hipotiroidismo, pero tiene su indicacin principal cuando se sospecha
hipertiroidismo, sobre todo en pacientes con TSHs (segunda o tercera generacin)
inhibida con T4 normal. Esta asociacin es conocida como hipertiroidismo por T3.9
La determinacin de T3 (libre y total) resulta de utilidad en otras situaciones, en las
cuales no hay concordancia con los valores de T4 (libre y total ) ni con la clnica, tales
como:

3. Los que abogan por la estrategia combinada se basan en las limitaciones expuestas
relacionadas con la determinacin de TSH.
Se concluye que, aunque no se ha logrado la prueba que sustituya a las dems, la
determinacin de TSH, como prueba inicial, es muy til en el diagnstico de la
disfuncin tiroidea y se debe indicar e interpretar con clara compresin de sus
limitaciones, teniendo siempre en cuenta la clnica y, de ser necesario, asociarla a la
determinacin de T4l o de T3l. El reto est en emplearla en el paciente y en el
momento indicado.
Summary
The thyroid diseases may be diagnosed in most of cases by the clinics. However, there
are circumstances that required thyroid function tests to correctly assess the patient in
initial stages of thyroid dysfunction and give an adequate treatment on an individual
basis and the follow-up of the disease. This review makes reference to the tests used
to assess the condition of the thyroid function and mentions their advantages,
limitations and present trends. The determination of TSH by second and third
generation procedures allow setting up diagnoses of primary hyperthyroidism and
hypothyroidism, even in the subclinical forms of both entities since or hypersecretion
or inhibition are respectively exposed, and they also allow prescribing the right dose of
1-Levothyroxine and orienting towards a hypophyseal cause of thyroid dysfunction. T4
determination is of great value for finding out the intensity of thyroid dysfunction and
furthermore assessing the effectiveness of the treatment in the first weeks. T3
determination is mainly indicated when the so-called T3-caused hyperthyroidism is
suspected. The determination of total T4 and T3 gives rise to a wrong interpretation
regarding the conditions that change the thyroxine-transporting globulin (TBG). The
present trend is the use of TSH as an initial test for diagnosing the thyroid dysfunction,
which should be indicated taking the clinic into account, and if necessary, it should be
associated with the determination of free T4 or free T3.
