Transforming Care

Transforming Care is about making health and
care services better for people with learning
disabilities who have mental health conditions or
behaviour which challenges.
Transforming Care is about helping people live
the best lives possible, in the right place and
with the support they need. NHS England wants
to ask people with learning disabilities and/or
autism and their family carers what they know
about Transforming Care. There are 5 parts to this
questionnaire.
If you could fill out this questionnaire and send it
back to Speakup. By the 18th of March

Local people with learning disabilities and family
carers should know about Transforming Care
and the role that Local Authorities and Clinical
Commissioning Groups have to play in making
sure it happens.

Changing how care and support happens
