Caso clnico
Presentamos el caso de una paciente de 68 aos con
diagnstico de fractura diafisaria del hmero derecho cerrada, sin lesin del nervio radial, secundaria a un traumatismo tras caerse desde su propia altura.
El examen radiogrfico evidenci una fractura en dos
fragmentos, desplazada en valgo, de trazo espiroideo, extendida desde el tercio proximal de la difisis hasta la
unin del tercio medio con el distal (Fig. 1).
Para su tratamiento se decidi utilizar la tcnica de osteosntesis mnimamente invasiva con placa helicoidal.
Tcnica quirrgica
Con la paciente bajo anestesia general en decbito dorsal, con el brazo levemente abducido sobre tabla, se realiz una incisin lateral transversal de 5 cm de longitud,
2 cm distal a la articulacin acromioclavicular. Con diseccin roma transdeltoidea longitudinal se expuso la insercin del manguito rotador a nivel del troquter. Se aplic un punto de sutura en cruz con Ethibond 5 al tendn
del msculo supraespinoso y se repararon ambos cabos
de la sutura.7,10 Se procedi de igual modo con el tendn
del msculo infraespinoso y el tendn del msculo subescapular.7 Se tom la precaucin de no integrar accidentalmente el tendn de la porcin larga del msculo bceps al
aplicar el punto de sutura en el tendn del msculo subescapular.

Posoperatorio
Al segundo da del perodo posoperatorio se indic
comenzar con ejercicios de movilidad activa evitando
tanto fuerzas de torsin como levantamiento de carga. A
la cuarta semana la paciente alcanz una amplitud satisfactoria de movilidad en el hombro y el codo (Fig. 4A,
B y C).
Cumplido el tercer mes de la operacin se autoriz la
actividad fsica irrestricta.
Discusin
El principal beneficio de la utilizacin de clavos endomedulares acerrojados en el hmero consiste en una diseccin quirrgica limitada.4,6,8,9,13,18 Sin embargo, varios
