SOME IMPORTANT POINTS

There are some points that everyone should know in order to avoid any trouble
1. As u know that our some teachers are doing two different Government jobs at two
different places, so they have two different stamps. So whenever u get yr papers attested
from any teacher, make sure that they use their IPS, UVAS stamp for attestation, otherwise
yr papers will be rejected.

2. The normal fee of 5 yrs DMCs verification is Rs. 850 and urgent fee is Rs.1700,
but I would advise that u people should apply on normal basis bcoz you have to
submit yr Original fee bank chalan form in PPC office and in most cases they send it
to uni after two weeks, so theres no point of urgent fee process.
3. Lastly, but most importantly, PPC send one set of attested DMCs along with original bank
chalan to our uni by post, there they get verified and then posted back to PPC office by uni
admin. So u shud be vigilant during the process bcoz in yr papers can get lost during this
posting process. Im saying this bcoz Ive faced the same problem and it was very difficult to
cope with
4. Make sure to get every important document photocopied during the procedure as u may
need it in the future

First of all I would like to congratulate all of u on this achievement
This would not have been possible without countless efforts of our admin, respected teachers and students
Now I will tell u the complete procedure of getting the category.
1. Go to website of Punjab pharmacy council and download the registration form and fill it.
2. Attach these documents with the form
3. 4 photographs attested by Dean/Chairman/Professor of the Faculty concerned
4. 2 Photostat copies of Degree attested by Dean/Chairman/Professor or the Faculty concerned (in our case
provisional certificate)
5. 2 specimen Signature duly attested by Dean/Chairman/Professor or the Faculty concerned (in both languages i-e
English and urdu)
6. 2 Photostat copies of National Identity Card attested by Dean/Chairman/Professor or the Faculty concerned
7. 2 Photostat copies of I, II, IIIrd, IVth and final year result Cards attested by Dean/Chairman/Professor or the
Faculty concerned
8. Registration Fee Rs. 2000/- (it will be paid in cash in PPC office)
9. Fee for Degree verification for as prescribed/required by the University Concerned (in our case the fee will be
only paid for verification of DMCs of five years, not for the verification of provisional certificate)
10. Character Certificate from any Professor or the Faculty ( attested copy)
11. An affidavit attested from Oath Commissioner/Notary Public/Grade 18 Officer on Non Judicial paper of Rs.20/indicating specimen overleaf (the specimen is written in the registration form that u will download from PPC
website, u just have to get it printed on Rs. 20 stamp paper. You can get it done from every where but Ill prefer
Chaudhary Law associates as their shop is located near our uni)
12. Attested Photo copies of Matric & FSc certificate (one copy each)
13. And the pass out certificate from the department mentioning our session i-e 2009-14
