Identified problems
A. Health Threats
Presence of breeding places for mosquitos and insects
It was observed that the family stores water in drums and
containers for which some of the stored water were evidently hazy. It
shows that it was stocked for days, showing some of pupa . The family
have chickens, they flock everywhere and their wastes were scattered.
Markedly, they unable to maintain the cleanliness of their
surroundings.
Poor Hygiene of Kids
It was observed that the children have dirty hand with
untrimmed nails. They ate without washing their hands. Their clothes
are stained, they just everywhere.
B. Health Deficit
Cough and colds
Their three little children were having cough and colds, and they
dont know any interventions . Additionaly, because of inaccessibiity to
health through transportation, they were not able to bring their
children for check-up.
C. Foreseeable Crisis
Poor environmental sanitation
It was observed that wastes of chicken are scattered. The door is
always open, flies and mosquitoes even chickens enter the house.
There is no proper waste segregation and drainage. Cooking places are
outside the house , plates or other clothes are just randomly placed
inside the house.
Improper garbage disposal
Prior to the conducted interview and observation , it was identified
that the family just burn their wastes and throw some of it everywhere,
which was observed, there is no certain pit for their garbage.
D. Second Level Assessment

The family was able to recognize their problems but due to


financial problems and inaccessibility to health, they are hesitant to do
it.
