1. Please fill out the PRC form in long bond paper, landscape orientation, computerized
using the Arial font size 10.
2. No abbreviations allowed for the diagnosis, operation performed, type of anesthesia,
name of hospital and the date.
3. Please refer to the Hospital OR-DR logbook for the data of the case that you need.
4. Class mayors will be informed when to submit the forms for checking.
5. Batch Advisers will do the checking of the cases
