PERFORMANCE DETAILS
Date of Performance

In the event Musicians cancel performance due to, accident, acts of God, or any other condition beyond their control (Force Majeure),
any deposits previously paid will be refunded. Client shall not hold Musicians or any Third Party liable. Musicians shall use best efforts
to provide a substitute, which shall be subject to approval by Client. Client has same rights of Force Majeure in which case, Balance
Due, shall be forgiven. Once musicians have left for the venue, Client no longer has the right of exercising Force Majeure.

Venue Information

If venue is outside or otherwise not climate controlled, upon their sole discretion, Musicians may delay or stop performing due to
unsafe conditions, and wait until the stated End Time for conditions to improve or measures taken that allow the performance to
continue. Force Majeure shall not apply and no payment shall be reimbursed to Client for shortened performance due to these
circumstances.

Setup By Time

The time by which Musicians shall have sound check completed and otherwise be ready to perform. Musicians will need access to
venue 2 hours prior to the stated time.

Start & End Time

Periodic breaks to be taken by Musicians or at the request of Client. Client or Musicians can provide pre-recorded music to be played
during breaks. Performance may be extended, at the discretion of the Musicians, at the rate of $200 per hour.
Musicians shall not be responsible for late Start Time caused by event delays. Performing past the stated End Time shall be at the
discretion of Musicians and at no obligation. After performance, Musicians shall be allowed 1 1/2 hours to tear down and vacate the
venue.

Wage Agreed Upon

Client may cancel this contract up to 90 days prior to the performance date without obligation of paying the Balance Due.
Cancellations with less than 90 days notice, require 100% of Balance Due be paid by the Client.

Required Deposit

Deposits are non-refundable. Any exceptions are at discretion of the Musicians. Postponements shall have deposit applied to a later
date, if date is available. Deposits may be in the form of personal or certified check (payable to XXXXX) and mailed to above
address. There will be a $30 charge for returned checks.

Balance Due

WITHIN 14 DAYS OF PERFORMANCE - PAYMENT OF BALANCE DUE SHALL BE MADE IN THE FORM OF U.S. CURRENCY,
CERTIFIED CHECK, or PERSONAL CHECK (payable to XXXX)

Musicians Attire
Formal

Formal (white shirt, vest, black pants, black shoes)


GENERAL INFORMATION

Client shall be responsible for providing the performance area. Minimum dimensions shall be 12 feet deep by 16 feet wide.
Client must ensure proper electrical access and capacity. For this performance 3 properly grounded, 15 or 20 amp, 110 volt circuits, with no other load on
them, shall be required. Outlets for all circuits shall be located at the performance area. Musicians cannot be held responsible for power interruptions. In the
event a power outage prevents all or part of a performance, no payment shall be reimbursed to Client.
Musicians shall be responsible for providing sound system and lighting for their performance. Unless otherwise agreed upon, Musicians have no obligation to
provide use of sound system to any other Third Party.
While at location, Musicians shall maintain control of performance area. This includes the right to restrict access to other persons as well as to remain at or
near performance area at all times. Any storage areas identified for equipment or personal property must be secure with access controlled solely by
Musicians.
Unless previously agreed to, there are no guarantees a specific song will be performed.

ADDITIONAL TERMS AND SERVICES


Master of Ceremonies Client will provide detailed itinerary, including a time line of events taking place. Any event specific music Musicians cannot
source should be provided by Client in the form of a CD to be forwarded at least 15 days prior to the event. It is recommended Client provide contact
person at event. In lieu of providing contact person, Musicians shall use best efforts in providing this service and use their discretion to keep event on
schedule.
Wireless Microphone - will be provided by Musicians for toasts, speeches, etc.
Pre-recorded music for Cocktails & Dinner If desired, In lieu of Client furnishing music in the form of a CD, Musicians will provide.
Client shall provide a dedicated location at the venue for Musicians to occupy prior to their performance. This can be a table in the
reception hall or an alternative room in the venue.
Client to provide (5) meals for Musicians meals can be boxed lunch type
Client agrees to indemnify and hold Musicians harmless from any and all claims, liabilities, damages, and expenses arising from any action or activity of
Client or Musicians while Musicians are rendering the contracted services except for claims arising from Musicians willful misconduct or gross negligence.
This contract constitutes a complete and binding agreement between the Client and the Musicians. The persons signing for Client and the Musicians agree to
be personally, jointly and severally liable for the terms of this contract. A fully executed scanned image or facsimile of this contract may serve as a valid
contract. Signed contracts should be mailed to the name and address in the upper right hand of contract.
