Instruction to Advertiser regarding change in name, religion and birth date

Instructions may be followed carefully before filling up this notice. Notice should be filled in Unicode font only.
Only one word should be written in each blank space on the first page of notice. Government accepts no
responsibility as to the authenticity of the content of the notice. Since they are based entirely on the
application of the concerned person without verification of documents. The notice should be filled up in
English only.

The publication in the Maharashtra Government Gazette of a notification regarding change of name, religion
or birth date confers no right on the person concerned to have the name, religion or birth date changed on
Government or other records. Similarly, Government does not take any responsibility, if the notice of change
of name, religion or birth date is given by any unauthorized person, since publication of such notices is an
advertisement and not an authentic record or proof for the name, religion or birth date, so changed.

Citizens are filling and uploading the notices on this office website dgps.maharashtra.gov.in on his/her own
responsibility, hence no correction is allowed in uploaded notices. In order to ensure correctness, notices of
change of name, religion or birth date should be filled up properly and completely in the provided space or
fields. This office is not responsible for any mistake. Offline notices along with demand draft/Money order are
not accepted. Filling up and uploading the notices regarding change in name, religion and birth date is
provided online, hence citizens need not to visit the counters of the Government Book Depot, Mumbai for
filling up the notice and also to collect the published gazette. Published gazette copies are not sent by post
and citizen can download the published copies from the above mentioned website.

Citizen should fill up the complete details in the online notice and take its print out, sign with old and new
name and upload the signed notice. Along with the notice it is compulsory to upload the scan any one of the
photo identity proof such as Aadhar card, Voters ID, Passport, Pancard, Driving license. Citizen should also
upload the relevant scanned legal document as per the reasons mentioned in the notice regarding change in
name, religion and birth date

Illiterate person should impress their left hand thumb impression on the print out of the completely filled up
online notice and then upload it.

In case of minors (below 18 years), completely filled up online notice print out should be countersigned by the
guardian/mother or father of the applicant and upload it. Along with the notice it is compulsory to upload the
scan any one of the photo identity proof such as Aadhar card, Voters ID, Passport, Pancard, Driving license
of the guardian and Birth certificate of the applicant.

Charges for General Notice: - Charges of general notice for change in name, religion and birth date are Rs
500 plus Rs 20 for Mahaonline charges and service tax extra. For backward class applicant Rs 250 plus Rs
20 for Mahaonline charges and service tax extra. Notice charges should be paid by only e-payment such as
through Debit card, Credit card, Net Banking system or cash at CSC center.

Backward/Scheduled caste applicants may upload attested scanned copy of caste certificate of old name
issued by the collector of the district concerned/Tahsildar. [(1) Scheduled Tribes, (2) Vimukta Jatis, (3)
Nomadic Tribes, (4) Navbuddhas, (5) Tribals outside the specified areas of Vidarbha and (6) Scheduled
casts, (7) Special other Backward Class are included in backward Class]. Other backward classes are not
considered for concession. In case of minors (below 18 years), Backward/Scheduled caste applicants, may
upload attested scanned copy of caste certificate of fathers old name issued by the collector of the district
concerned/Tahsildar.

Normally 10 to 15 days time is required for publication of the General advertisement in the Maharashtra
Government Gazette. Emails and sms are sent to respective applicant regarding Status of their notices.
