Leony, G5P4 was admitted at 1 pm today due to watery vaginal discharge.


The cervix was 3cm dilated, cephalic, (+) BOW. BP=120/80, PR=80/min, T36.5.
At 5pm, contractions were moderate, 3 in 10 min. IE showed cervix 4cm
dilated. Vital signs remained the same.
At 9 pm, your IE showed 6cm dilated cervix, BOW ruptured with clear
amniotic fluid.
At 1 am, another IE done showed 8cm dilated cervix, meconium stained
fluid. BP-110/70, PR-92/min, T-37.5, FHT-140/min.

Juana, a G3P2 was admitted today at 4 pm. IE showed a 6cm dilated


cervix, cephalic, intact BOW. BP=110/80mmHg, PR=90bpm, afebrile.
FHT=135bpm.
She had moderate contractions (4 in 10mins.). At 8:00pm, the BOW
ruptured with clear amniotic fluid, IE showed a full dilated cervix. Vital signs
were the same.
At 8:30 pm, she delivered spontaneously to live baby boy. 10U oxytocin
was given IM at the deltoid muscle. Placenta was delivered completely at
8:45pm.
