PETITION
COME NOW, the Petitioner, unto this Honorable Board, respectfully
alleges:

1. That the petitioner is a grantee of a Certificate of Public Convenience

to operate a FILCAB Ordinary Regular service for the transportation of
passengers and freight along the route:

2. That said grant of Certificate of Public Convenience to herein petitioner
was issued by this Honorable Board through a Decision dated

entitled (name of petitioner), Application for a Certificate of Public
Convenience to operate a FILCAB Ordinary Regular service under the
Colorum Legalization Program;

3. That said Certificate of Public Convenience per Decision dated
_________ is valid and subsisting for a period of 5 years counted from
said date and thus will be expiring in ____________
4. That petitioner therefore requests that the Certificate of Public
Convenience granted to her be renewed;
5. That petitioner is financially capable to operate and maintain the
proposed service;
6. That there is still an urgent need for the proposed service;
7. That Petitioner shall continue to abide by the rules and regulations
relative to public transportation operation.
8. That in support of this petition for renewal of Certificate of Public
Convenience, petitioner hereby attaches the following documents:
a. Income Tax Return
b. Tax Identification Number
c. Audited Financial Statement
d. Certificate of bank deposit and passbook
e. OR/CR of authorized unit/s with year model
f. Latest copy of decision and/or order
g. Franchise Verification
h. Proof of Filipino Citizenship
i. Location map and picture of garage with dimensions, TCT/tax
declaration Clearance of accounts including LTFRB Official Receipt
j. Passenger Accident Insurance
k. Community Tax Certificate
l. 2 copies of passport size (2 x 2) picture with specimen signature at
the back
m. Information Data Sheet
WHEREFORE PREMISES CONSIDERED, it is most respectfully prayed
that this Petition for Renewal of CERTIFICATE OF PUBLIC CONVENIENCE
to operate a FILCAB Ordinary Regular service be approved.
