with law, hereby depose and say:
1. That my name appearing in my Certificate of Live Birth issued by the National Statistics
Office (NSO) is MA. SOLEDAD VILLEGAS MALOLES, as shown by a copy of the
said document hereto attached as Annex A;
2. That however, I have been using the name/S MYRNA V. MALOLES and MA.
SOLEDAD MYRNA M. BELLEZA, in my Marriage Contract and Senior Citizens ID,
a copy of the said documents using such name are hereto attached as Annexes B and
C;
3. That the names MA. SOLEDAD VILLEGAS MALOLES, MYRNA MALOLES
BELLEZA and MA. SOLEDAD MYRNA M. BELLEZA refer to one and the same
person, which is the undersigned/affiant herein;
4. That I am executing this affidavit to attest to the truthfulness and veracity of the
foregoing and for whatever legal purpose it may serve best.

2. That we personally know MYRNA MALOLES BELLEZA since the 1970s because we
are family friends and we have known her for more than 30 years;
3. That we also personally know that MYRNA MALOLES BELLEZA has been using that
first name since she was a child;
4. That we attest that MA. SOLEDAD VILLEGAS MALOLES, MYRNA MALOLES
BELLEZA and MA. SOLEDAD MYRNA M. BELLEZA is one and the same person;
5. That we are executing this affidavit in order to attest to the truth of the facts
