Submit this form and all required documentation to update identifying information in your MCAT file for past exams and/or
to your AAMC ID account. Required documentation may be a copy of an original document and must be notarized. Please
contact AAMC Services at (202) 8280600 or MCAT@aamc.org for additional assistance.
IMPORTANT: For upcoming MCAT exams, you are responsible for ensuring that the first and last name(s) on your ID exactly
match the names with which you are registered. If you are registered using a partial name, nickname, or any deviation of
the name that appears on your official ID (including misspelling), you will not be admitted to the testing room.
Login to the MCAT Scheduling and Registration System prior to the Bronze Zone deadline to submit any changes to your
registered name.
NAME CHANGE
A significant name change requires a legal court or government document displaying evidence of the name change and a
notarized copy of your governmentissued ID.
A name change due to an error (e.g. a partial name, nickname, misspelling, and/or use of a hyphen) requires a notarized
copy of your governmentissued ID.
DATE OF BIRTH
A notarized copy of a government issued ID that clearly displays the date of birth is required.
SEX
A sex designation change does not require documentation and may be done upon request in writing.

The provision of false or misleading information is grounds for disciplinary action, including, but not limited to, withholding
or voiding of an examinees MCAT scores. Furthermore, an investigation into the circumstances may result in reports of the
incident that may be sent to institutions to which the examinee(s) in question has applied for admission.
