11 Do you have off- hours duty personnel. How about in the office. Do
they have contact number.

issued. Are there any activities that promote teamwork among
personnel?

5. Is there any Legislation or Circulars by Authority like PEOA,


Marina, PRC, MTC etc.. How do you obtain such a copy. Is it
available and ( posted on bulletin board) disseminated to all
concerned?

3. Are personal records for all crew maintained, contact numbers and
filing system updated? Is it available upon request?
4. Who is evaluating the documents and conducting interview. Do
you have procedure/checklist followed?
5. Does the company verify the authenticity and validity of
documents? How it is done?
6. Are Medical tests conducted to all joining crew? Is a list of
accredited Medical clinics available? Records of medical test
available and filed?
7. Instruction/briefing given to joining seaman. Travel information
and documents given to crew ahead of time?

8. For new recruit, is background check conducted? Is it an SOP?

9. Proficiency of seamans English language checked.

10. Is pre-embarkation orientation given to seafarer
