PENDAHULUAN

FAKULTAS KEDOKTERAN GIGI

UNIVERSITAS GADJAH MADA
2015

BAB I
LATAR BELAKANG

memeriksa pentingnya kesan pertama.
School of Dentistry dalam populasi umum.

BAB II
PEMBAHASAN

A. PENDAHULUAN

Selama beberapa tahun, profesionalisme dalam perawatan kesehatan memiliki beberapa

penegertian, beberapa diantaranya memiliki makna yang rancu/ambigu. Profesionalisme

keseluruhan 64% nya kurang terkover asuransi dental. Berikut penjelasan tambahan

blus, celana panjang, kemeja).

mahasiswa KG sangat mempengaruhi kepercayaan mereka tehadap kemampuan
mahasiswa KG.

Ketika ditanya tentang karakteristik fisik anggota fakultas kedokteran gigi (Tabel 5),
56 % pasien sangat setuju bahwa penampilan anggota fakultas kedokteran gigi
dicontohkan integritas profesional.

Hampir 73% pasien setuju (44% sedikit
setuju, 29% sangat setuju) bahwa penampilan / pakaian anggota fakultas kedokteran
gigi ini sangat mempengaruhi tingkat kecemasan mereka, sementara temuan serupa
ditunjukkan ketika pasien diminta tentang tingkat mereka kenyamanan.

mereka mengenai tingkat profesionalisme anggota dental staff.

membuat perjanjian.

Abstract: The purpose of this study was to examine how patients perceived the professionalism of University of Minnesota
School of Dentistry students, faculty, and staff. Professionalism is defined by the authors as an image that will promote a
successful relationship with the patient. Patients within comprehensive care clinics were asked to assess physical attributes and
behaviors of the dental care providers using a questionnaire. The patients read statements dealing with characteristics of the dental
care providers and responded as to whether they agreed, were neutral, or disagreed with the statement. The surveyed population
consisted of 103 males and 97 females, 64 percent of whom lacked insurance coverage. Fifty-one percent of the patients were
between the ages of forty-four and sixty-nine, but the overall age distribution was dispersed over a range of eighteen to one
hundred. Our research found that all dental care providers displayed a professional appearance as well as behavior. The attire of
the dental care provider affected the comfort and anxiety levels of patients, as did first impressions of both students and faculty.
Most patients reported that students and faculty displayed effective time management and used appropriate language during the
appointment. Finally, hairstyle, makeup, and jewelry appeared to have little effect on patients opinions of the various dental care
providers.

ver the years professionalism in health care

has had several definitions, many of which
are very ambiguous.1 Professionalism is expressed in various ways including the dimensions of
competence, ranging from comprehension of basic
biology principles to clinical skill; engagement,
which includes behaviors and attributes pertaining
to empathy and communication; reliability, which
pertains to timely access to competence; dignity,
which includes treatment of patient, clinical staff, and
self; the health care providers dedication to placing
the needs of the patient above his or her own needs;
and concern for the quality of clinical care.2 Three
broad components of professionalism were introduced by Walsh, suggesting that proper appearance,
behavior, and conversation are essential for a successful relationship.3 This study also found that patients are more apt to discuss medical issues with a

August 2003

Journal of Dental Education

health care provider who is well groomed, has a professional tone of voice, and a confident expression.
Physician attractiveness also positively relates to increased patient willingness to disclose symptoms.4,5
For simplicity, we define professionalism in its broadest sense as an image that promotes a successful relationship with the patient. This relationship would
be one in which the patient feels confident in the
capabilities of the health care provider.
A substantial amount of research dealing with
the professional image of health care providers has
been produced, largely focused on family physicians
or nurses. The recent available literature has shown
that patients tend to prefer a traditionally dressed
physician in contrast to a more casual appearance.6-8
As reported by Molloy,9 the most desirable outfit for
male physicians was a white coat with slacks, shirt,
and tie, while for female physicians the preferred

909

attire included a white jacket, skirt, and dark wool

shirt. Although additional studies undertaken by
Mangum et al.10 generated similar conclusions, the
question of exactly how important this professional
image is when it comes to the field of dentistry remains unclear. Therefore, the objective of this study
was to determine if professional image ideals observed within the medical field pertain to dental care
providers.
Our research focused on the importance of patients perceptions of their dental care provider based
on their physical appearance, attire (dress), behavior, and overall level of professionalism. Psychologists and sociologists have often stated the importance of physical appearance and its effect upon first
impressions and interpersonal relationships.11 As reported by Kanzler and Gorsulowsky6 and Gjerdingen
and Simpson,12 a carefully dressed physician may
convey the image of a meticulous, knowledgeable,
and compassionate individual, whereas an unkempt
appearance may portray disorganization and an uncaring attitude. Mangum et al.10 and Taylor,13 also
examining the importance of first impressions, found
patients initial perceptions influenced their expectations of care as well as their opinions of their
physicians competence level. These nonverbal cues
can play a significant role in interactions between
the patient and medical provider in addition to influencing the patients comfort, anxiety, and confidence
levels.
Our research project was undertaken to improve
the quality of care delivered to patients visiting the
University of Minnesota School of Dentistry and
within the general population. By examining how
patients perceive dental students, faculty, and staff
based on their level of professionalism, we hoped to
obtain a better understanding of the dynamics of the
patient/provider relationship. A comparative analysis of the research conclusions observed within the
medical field was a secondary goal of our study. The
final objective of this research study was to investigate how demographic variables such as gender, age,
insurance, and distance traveled to attend appointment affected patient survey responses.

Materials and Methods

Questionnaires were offered to patients following completion of their dental appointment in the
senior comprehensive care clinic at the University

910

of Minnesota School of Dentistry during a three-week

period in 2002. Questionnaires were offered to all
patients to decrease the potential for unwanted bias.
The senior comprehensive care clinic was chosen for
this investigation due to its variety of patients and
dental procedures performed (periodontics, prosthodontics, and restorative dentistry). Human subject
approval was obtained from the Institutional Review
Board of the Academic Health Center, University of
Minnesota. Informed consent was obtained from all
patients prior to enrollment.
To aid in the development of our questionnaire,
previous surveys were reviewed, and questions that
addressed our concerns at the University of Minnesota were selected.6,8,10,12 The questions were adapted
to reflect appearance, language, time management,
and professionalism. Questions were kept to a minimum to ensure that they would be completed. The
questionnaire contained thirty statements and used a
5-point Likert scale to rate dental faculty, student,
and staff professionalism (1=strongly disagree,
2=slightly disagree, 3=neutral, 4=slightly agree,
5=strongly agree).10 Patients were asked to rate the
dental students, faculty, and staff based on physical
characteristics (attire, facial hair, makeup) and behaviors (time management, use of technical language). Patients were also requested to provide their
perceptions of the overall treatment process including such issues as comfort, anxiety, confidence in
the care provider, and first impressions. Additional
information pertaining to patient demographics, such
as age, gender, insurance coverage, and distance traveled to attend the dental appointment, was also collected.
Completed patient questionnaires were analyzed for response frequency and the results were
tabulated. Cross-tabulation analyses (Pearson) were
performed between each demographic variable and
each questionnaire statement.

Results
Of the 208 patients asked to participate, six
refused due to personal time constraints, and two did
not completely fill out the questionnaires demographic section. The surveyed patient pool consisted
of 103 males (51 percent) and 97 females (49 percent), with 128 patients (64 percent) lacking dental
insurance coverage. Additional demographic data
including patient age group distribution and the dis-

Journal of Dental Education Volume 67, Number 8

tance traveled by each patient to reach their dental

appointment are provided in Table 1.

Dental Student Results

Of the thirteen survey questions used to describe dental students, five pertained to physical attributes, four dealt with behaviors, and four explored
the resulting patient perceptions following treatment.
When asked about physical characteristics of dental
students (Table 2), 56 percent of the 200 surveyed
patients strongly agreed that the students appearance
exemplified professional integrity. The majority of
patients (49 percent) provided neutral responses as
to a preference for either a white lab coat or surgical
scrubs, and 48 percent remained neutral when asked
about hairstyle, makeup, and jewelry. Concerning
formal dress attire, over half of the patients agreed
(34 percent slightly agree, 21 percent strongly agree)
that dental students should be required to wear dress

slacks, collared shirts, skirts, or blouses. When asked

if casual dress attire was acceptable for a dental student, most patients (44 percent) were neutral, and
roughly one-third said no (27 percent slightly disagreed, 5 percent strongly disagreed).
When examining behavioral characteristics
(Table 3), over 80 percent of patients agreed that dental students displayed effective time management, and
two-thirds of patients strongly agreed that students
used appropriate language. Patients reported student
efforts at maintaining an orderly work environment
were effective and said that students displayed a level
of professionalism comparable to that of the dental
faculty. Overall, nearly 93 percent of patients indicated that dental students portrayed an equal level
of professionalism to the faculty.
Four questions were asked to examine patient
perceptions of the dental student following treatment
(Table 4). Over half of the patients strongly agreed
that their first impressions of the dental student
greatly affected their confidence levels in his or her
abilities. Nearly 80 percent of patients agreed that
the students appearance/attire affected their feelings
regarding the care they would receive in the future.
It was also reported that roughly one-half of the patients strongly agreed that appearance/attire greatly
affected their comfort and anxiety levels; less than 3
percent strongly disagreed.

Note: Two patients failed to respond when asked for

gender.

Dental Faculty Results

Of the ten survey questions used to describe
the dental faculty, two pertained to physical attributes,
four dealt with behaviors, and four explored the resulting patient perceptions following treatment. When
asked about the physical characteristics of faculty
members (Table 5), 56 percent of patients strongly
agreed that the faculty members appearance exemplified professional integrity. Concerning formal
dress attire, over three-fourths of the patients agreed
(33 percent slightly agree, 45 percent strongly agree)
that a collared shirt and tie along with dress slacks
or a skirt and blouse were appropriate attire for the
faculty.
When investigating behavioral characteristics,
over 84 percent of patients agreed that the dental faculty displayed effective time management (Table 6),
and 73 percent of patients strongly agreed that the
faculty members behavior portrayed professional
integrity. The dental faculty also received favorable
responses when patients were asked to rate the use
of language both with the students and the patient.

Four questions were asked to examine the resulting patient perceptions following treatment with
the faculty member (Table 7). Nearly 90 percent of
patients agreed (45 percent slightly agree, 44 percent strongly agree) that their first impressions of
the faculty member greatly affected their confidence
levels in his or her expertise. Only 3 percent of patients disagreed that the dental faculty members attire inspired confidence in his or her abilities. Nearly
73 percent of patients agreed (44 percent slightly
agree, 29 percent strongly agree) that the dental faculty members appearance/attire greatly affected their
anxiety levels, while similar findings were shown
when patients were asked about their levels of comfort.

Additional Results
Patients were asked three questions pertaining
to the behaviors and physical attributes of the dental
staff (Table 8). Nearly two-thirds of patients strongly
agreed that the staff members appearance and behaviors portrayed professional integrity, and the ma-

jority of patients (55 percent) remained neutral on

whether casual attire was appropriate for the dental
staff. When asked about staff members hairstyle,
makeup, or jewelry selections, 37 percent of patients
were also neutral.
When analyzing the relationships among age,
gender, insurance coverage, and travel distance to
appointment with the various survey questions, we
observed several significant findings. Female patients
were more likely to agree that hairstyle, makeup, or
jewelry played an important role in their opinion of
the dental staff members level of professionalism
(Pearson p-value = 0.010). Patients who traveled less
than ten miles to attend their appointment had a tendency to agree that formal attire (collared shirt and
tie, dress slacks, skirt and blouse, etc.) was appropriate for the dental faculty (p = 0.006). This same
group of patients was more likely to agree that formal attire should be required for dental students (p =
0.001) and to disagree that casual attire (tennis shoes,
jeans, t-shirt, etc.) was acceptable dress for a dental
student (p = 0.010). Based on the previous three observations, it appears that patients traveling from
greater distances to attend their dental appointment
were more accepting towards both faculty members
and dental students wearing casual attire.

Discussion
The purpose of this study was to explore patients perceptions of professional attire and behav-

ior of dental care providers as indicators of the professional integrity and competence within the University of Minnesota School of Dentistry. For purposes of this discussion, we defined professional
integrity as the ability of the health care provider to
be worthy of trust, have concern for others, and have
a substantial commitment to the patient. Professional
competence is the health care providers ability to
provide adequate dental care. We found both similarities and differences in comparison to the findings of studies of professional image among physicians and nurses. The general patient preference
toward traditional/formal physician dress observed
by Kanzler and Gorsulowsky,6 McNaughton-Filion
et al.,7 and Gjerdingen et al.8 was confirmed by our
data. Over 75 percent of the patients agreed that formal attire was appropriate for faculty members, and
over half agreed that dental students should be required to wear dress slacks, collared shirts, skirts, or
blouses. Despite a recent movement in Western culture toward a more relaxed, casual attire in the general public and within the office setting, it appears
that patients continue to expect a formally dressed
dental health care provider. When asked if casual attire was acceptable for a dental student, 44 percent
of patients were neutral, but 32 percent disagreed,
further emphasizing the patient preference towards
formal attire within the dental setting.
Patient responses in this study were somewhat
different from the findings of several previous studies concerning the traditional patient preference for
physicians to wear a white laboratory coat. When

913

patients were asked whether dental students should

wear a white laboratory coat rather than a surgical
smock or scrubs, the distribution of responses covered a wide range of opinions. The majority of patients (49 percent) remained neutral, while the remaining patients either agreed (24 percent) or
disagreed (27 percent) with the questionnaire statement pertaining to the white laboratory coat. These
results perhaps hint that patient preference for the
traditional white laboratory coat has decreased.
As described by Taylor,13 the patients first impression of a physician can have a strong influence
on perceptions of the care providers competence level
and expectations of the care delivered in the future.
We found the attire/appearance of both students and
faculty had a strong influence on the patients comfort and anxiety levels, and patients agreed that their
first impressions of faculty and students affected their
confidence level in both groups. The first impression appeared to play a stronger role in how patients
perceived students in comparison to faculty members, possibly due to differences in the expectations
of experience/expertise between the two groups.
The patients responses were largely neutral for
attire and appearance relating to students, faculty, and
clinical staff. This indicates that appearance, or at
least the specific elements of it, mentioned in our
survey questions is not the only factor that provides
confidence. The way health care providers verbally
treat their staff, the amount of perfume worn, and
their listening skills may mean more to a patient than
if they are wearing a scrub suit and tennis shoes or a
suit and tie. Professionalism is multi-factorial and is
important in engendering a sense of comfort and
confidence during treatment. Although first impressions are important, communication skills, behavior, and attitude may have a greater impact on the
overall relationship between the patient and the health
care provider.
The nonverbal communication attributed to a
dental care providers attire/appearance can also play
a major role in the patient/doctor relationship. Positive first impressions, traditional dress, and an overall level of professionalism portrayed by the dental
health care provider can increase patient feelings of
comfort and confidence, thus leading to an improved
experience within the dental setting. These positive
patient attitudes can lead to an increase in exchange
of information between the patient and the dental
health care provider, in turn leading to more efficient and improved delivery of dental care.

Conclusions
The primary conclusions of our study are as
follows.
1. Dental students, faculty, and staff of the University of Minnesota School of Dentistry were all
considered to have a professional appearance as
well as behavior.
2. Patients preferred a more formal dress attire for
both dental students and faculty.
3. For dental students, attire greatly affected both
the comfort and the anxiety levels of the patients.
These results were observed, but to a lesser degree, for faculty.
4. Hairstyle, makeup, and jewelry appeared to have
little effect on the patients opinions of the students, faculty, and staff.
5. First impressions of the dental students appeared
to have a greater effect on confidence levels of
patients than did first impressions of the faculty.
6. Patients strongly agreed that dental students and
faculty members used appropriate language and
displayed effective time management skills during their appointment.
