Newly hired teachers for FY 2015 are advised to submit the needed documents for the processing of salary. Submission of documents is on or before November 27, 2015 at the Personnel Section. Incomplete documents will not be accepted. 2. Here are the documents to be submitted, FOR FIRST PAYMENT OF SALARY (for newly created positions/newly-flled positions) 3 copies of signed appointment with stamp received by the CSC (c/o DepEd Personnel Section) 3 copies of Oath of Office 3 copies of First Day of Service 3 copies of Disbursement Voucher 3 copies of SALN Copy of Form 7 MDR No. (PHILHEALTH) HOME (PAGIBIG) No. BIR Form 2305 or 1902 BP No, (SIS) (c/o DepEd Personnel Section) FOR SUCEEDING SALARY CLAIMS: * 3copies of Appointment #3 copies of DTR (1 original, 2 photocopies)/Form 7 #3 copies of Disbursement Voucher ADDITIONAL DOCUMENT FOR NATURAL VACANCY: ‘© Approved Appointment of Incumbent (Promoted) ‘* Approved documents of incurnbent (Retired/Resigned and Transferred Personnel) 3. Also, substitute teachers are advised to submit the same documents for processing immediately after their last day of service.
