Secondary caries are also caused by the action of acids produced by bacteria.
Most common microorganisms with secondary caries are: Streptococcus mutans,
Lactobacilli, Actinomyces viscosus. Acid production from the bacteria is predisposed
by the penetration of bacteria along the tooth restoration interface (Garg, 2013).
The carious process involves destruction of dental tissues as a result of
microbial action. An untreated carious lesion may invade the oor of the pulp
chamber or extend along the root, resulting in perforation of the root. Caries are not
treated will lead to increase in size and can reach the pulp, which is a cavity in the
tooth that contains the nerve tissue and blood vessels. When it reaches the pulp
chamber, there will be a process of inflammation that causes pain throbbing.
Eventually, a bacterial infection can cause tissue death in the pulp chamber and the
infection can spread to the alveolar bone tissue, so it can happen abscess (Burkhart,
2008).

In this case already involves enamel caries and dentin, where only left behind a layer
of dentin covering the floor of the pulp chamber perforations or has occurred pulp /
pulp, causing inflammation of the pulp, caries is called deep caries with perforation.
Usually feels pain all of a sudden without any stimulation and for further care will be
much longer than in other caries (Yanagisawa, 2006).
