Important

In case of Individual accounts, please provide copy of latest ID document along with the request.
In case of non individual account, letter / board resolution to be submitted along with the request.
Scheme / product code change from Savings to Savings, Current to Current & Overdraft to Overdraft is permitted.
Transfer of account between scheme / product types is not permitted i.e. Savings to Current or Overdraft is not permitted.
The Scheme / product code of account will be changed to the new Scheme / product code subject to availability of new Scheme / product code at the
account Branch.
All the facilities extended to the account in the existing Scheme / product code will continue subject to the eligibility of the new Scheme / product code.
In case of Activmoney (sweep) facility in the account, the threshold limits for sweep-in and sweep-out will be modified as per the threshold limits of the new
Scheme / product code. Please get in touch with your RM to inquire about the threshold limits for the product variant opted by you
Bank may freeze account for the period required to complete account scheme change process .. Customer will not be able to transact in the account in the
interim period.

Declaration
I / We have read and understood the terms and conditions related to the change in account scheme with Kotak Mahindra Bank Ltd. and those relating to the
various services. I / We accept and agree to be bound by the said terms and conditions including those excluding / limiting the Banks liability. I / We understand
that the Bank may at its absolute discretion, discontinue any of the services completely or partially or vary the terms without any notice to me / us. I / We agree
that the Bank may debit my account for service charges as applicable from time to time. I / We declare and confirm that the same operating instructions and
nomination will apply for account in the new Scheme / product code. I / We authorize the Bank to link all the existing Phone Banking, Net Banking, Debit Card
facilities on my / our existing account to my / our account in the new Scheme / product code. I/We also understand that this application will be deemed as an
account opening document with the Bank.
