Abstract
Aim: This study examined how halitosis, oral dryness and
general oral health were impacted during treatment with
the Invisalign system. Furthermore, the effect of a lowdose chlorhexidine solution (CHX) was evaluated.
Patients and Methods: Thirtyone patients with good periodontal health participated in this crossover study and were
divided into two groups (group 1: CHX/no CHX, group 2:
no CHX/CHX). The following parameters were recorded
during the first 8 months of Invisalign treatment: stimulated saliva flow rate, organoleptic index, tongue coating
index, measurement of the oral volatile sulfur compound
level (ppb), modified gingival and plaque index and bleeding on probing index. Professional oral cleaning was performed at the beginning of each period lasting 3 months.
The patients received a questionnaire at the first, third,
fourth, sixth and eighth control visits.
Results: The very low volatile sulfur compound level was
significantly decreased by CHX (0.06%) during the first examination period (p = 0.02), i.e. for the first group of patients only. Neither halitosis, nor oral dryness, nor high
plaque or gingival index measurements were observed.
Oral health-related quality of life was hardly influenced by
wearing aligners and oral hygiene habits were very good.
Conclusions: This study provides evidence that Invisalign
treatment is characterized by only minimal impairment of
overall oral health and the associated quality of life. Consequently, it appears unnecessary to recommend the general

Zusammenfassung
Ziel: Es sollte untersucht werden, ob sich whrend einer
Behandlung mit Invisalign das anekdotisch geschilderte
subjektive Vorhandensein von Halitosis oder Mundtrockenheit objektivieren lsst und inwieweit sich die Mundgesundheit und die Lebensqualitt im Therapieverlauf verndern. Zustzlich wurde der Einfluss einer niedrig dosierten Chlorhexidin-Mundspllsung (CHX) ermittelt.
Patienten und Methodik: Es wurden 31 parodontal gesunde
Probanden in diese Cross-Over-Studie aufgenommen und
in zwei Gruppen aufgeteilt (Gruppe 1: CHX/kein CHX,
Gruppe 2: kein CHX/CHX). Zu acht Untersuchungszeitpunkten whrend der ersten 8 Monate der InvisalignBehandlung erfolgte die Erhebung folgender Parameter:
stimulierte Speichelflierate, organoleptischer Index, Zungenbelagsindex, Messung der oralen flchtigen Schwefelverbindungen (ppb), modifizierter Gingiva-Index, modifizierter Plaque-Index und Bleeding on Probing Index. Zu
Beginn der beiden 3-monatigen Untersuchungsperioden erfolgte zudem eine professionelle Zahnreinigung. Die Fragebgen wurden am ersten, dritten, vierten, sechsten und
achten Kontrolltermin beantwortet.
Ergebnisse: Durch die 0,06%ige Chlorhexidin-Gabe konnte
eine statistisch signifikante Reduktion der sehr niedrigen
Werte der flchtigen Schwefelverbindungen whrend der
ersten Periode (p = 0,02), also nur bei der ersten Gruppe,
festgestellt werden. Bei den untersuchten Patienten wurden
weder Halitosis, noch Mundtrockenheit, noch erhhte Plaque- oder Entzndungswerte beobachtet. Die mundgesundheitsbezogene Lebensqualitt wurde durch das Tragen der
Aligner kaum beeinflusst. Die Mundhygieneangewohnheiten waren sehr gut.

Introduction
Bad breath is seen as a taboo subject nowadays. There are
hardly any reliable data on its epidemiology and prevalence
in Germany [7]. In principle, 8590% of halitosis cases have
an oral cause, while the number of volatile sulfur compounds, especially the proportion of hydrogen sulfide,
methyl mercaptan and dimethyl sulfate, is increased [27,
35]. These substances are fermented, above all by gramnegative bacteria in the mouth, particularly in the area of
the dorsum of the tongue. There can be a positive correlation between true, demonstrable halitosis or a purely
subjective perception of pseudo halitosis and the sensation of a dry mouth. The oral mucosa dries out when the
saliva flow rate is altered or reduced. The microbiological
equilibrium of oral flora is no longer in balance, which
means increased levels of volatile sulfur compounds can develop [16].
Patients treated with Invisalign anecdotally describe
the subjective perception of halitosis and additionally an initial feeling of dry mouth.
The Invisalign system was developed in 1999 by Align
Technology Inc. (Santa Clara, CA, USA). The aim is to move
the teeth into the desired position using several consecutive
splints. The use of flexible appliances to move the teeth dates
back to 1945 when Dr. H. D. Kesling [15] described such a
method. Align Technology has brought Keslings approach
into the modern age by combining it with innovative CADCAM technologies and special laboratory techniques [13].
As the Invisalign splints ( = aligners) are supposed to be
worn for 2022 hours a day, if possible, and therefore lie on
the marginal parts of the attached gingiva for a very long period of time, the resulting potential risks to periodontal and
oral health have already been investigated in previous studies [25, 26].
Owing to its bacteriostatic and bactericidal properties,
chlorhexidine has been used for years as an antibacterial
medication to reduce plaque and microbes [1, 3, 9, 12, 18],
especially during orthodontic treatment. In addition, it demonstrably improves halitosis by reducing the levels of volatile sulfur compounds and reducing the pH of the dorsum of
the tongue [6, 28, 3135, 38].
Einleitung
Mundgeruch wird heutzutage oft als Tabuthema angesehen. Zuverlssige Daten zu Epidemiologie und Prvalenz
in Deutschland gibt es kaum [7]. Prinzipiell ist Halitosis zu
8590% oral bedingt, wobei die Zahl der flchtigen Schwefelverbindungen, vor allem der Anteil an Schwefelwasserstoff, Methylmercaptan und Dimethylsulphat, erhht ist
[27, 35]. Fermentiert werden diese Stoffe vor allem von
gramnegativen Bakterien der Mundhhle, besonders im
Bereich der dorsalen Zunge. Eine echte, nachweisbare
Halitosis oder eine rein subjektiv empfundene PseudoHalitosis knnen mit einem Mundtrockenheitsgefhl positiv korrelieren. Bei vernderter oder verminderter Speichelflierate sind die oralen Schleimhute ausgetrocknet.
Das mikrobiologische Gleichgewicht der Mundflora ist
nicht mehr in Balance, so dass vermehrt flchtige Schwefelverbindungen entstehen knnen [16].
Anekdotisch beschreiben Patienten, die mit Invisalign
behandelt werden, das subjektive Empfinden von Halitosis
und zudem ein initiales Mundtrockenheitsgefhl.
Das Invisalign-System wurde 1999 von der Fa. Align
Technology Inc. (Santa Clara, CA, USA) entwickelt. Mit
mehreren aufeinanderfolgenden Schienen sollen die Zhne
in die gewnschte Position verschoben werden. Der Gebrauch von flexiblen Apparaturen zur Zahnbewegung
reicht bis ins Jahr 1945 zurck, als Dr. H.D. Kesling [15]
solch ein Verfahren beschrieb. Align Technology fhrte
Keslings Ansatz in Kombination innovativer CAD-CAMTechnologien und spezieller Labortechniken in die moderne Zeit ein [13]. Da die Invisalign-Schienen ( = Aligner)
nach Mglichkeit 2022 Stunden getragen werden sollen
und somit einen sehr langen Zeitraum den marginalen Anteilen der Gingiva propria aufliegen, wurden in der Vergangenheit bereits durch Pilotstudien das daraus resultierende
potentielle parodontale Risiko und die Mundgesundheit
untersucht [25, 26].
Chlorhexidin als antibakterielles Pharmakum wird aufgrund der bakteriostatischen und bakteriziden Eigenschaften
seit Jahren zur Plaque- und Keimreduktion [1, 3, 9, 12, 18],
insbesondere whrend einer kieferorthopdischen Behandlung, benutzt. Darber hinaus fhrt es nachgewiesenerma-

en zur Besserung von Halitosis durch die Reduktion flchtiger Schwefelverbindungen und durch die Senkung des pHWertes der dorsalen Zunge [6, 28, 3135, 38].
Die Ziele der vorliegenden Arbeit waren
das anekdotisch geschilderte subjektive Vorhandensein
von Halitosis whrend der ersten 8 Monate einer Invisalign-Behandlung zu objektivieren und die Auswirkungen
einer niedrig dosierten Chlorhexidin-Mundspllsung auf
eine mgliche Halitosis darzustellen. Hierzu dienten zwei
standardisierte Indizes und die Messung der flchtigen
Schwefelverbindungen;
die orale und parodontale Situation der Probanden mittels eines Plaque-, zwei Entzndungsindizes und mit Hilfe
der Messung der Speichelflierate zu erfassen und den
Einfluss der Chlorhexidin-Mundspllsung hierauf zu untersuchen;
durch Fragebgen mehr ber die Mundgesundheit, die
mundgesundheitsbezogene Lebensqualitt, die Mundhygieneangewohnheiten, die Motivation und die soziale Integritt im Verlauf der Invisalign-Behandlung herauszufinden.
Patienten und Methodik
Es wurden 31 parodontal gesunde Probanden (7 mnnliche,
24 weibliche) in die Studie im Cross-Over-Design aufgenommen und alternierend in zwei Gruppen aufgeteilt. Alle
Patienten waren krperlich gesund, wiesen keine relevanten Allergien auf und nahmen weder Antibiotika noch
plaque- und speichelproduktionshemmende Medikamente
ein. Es gab zwei Untersuchungsperioden von je 3 Monaten.
Zwischen diesen Perioden lag eine 2-monatige Wash-outPhase. Die erste Gruppe wurde whrend der ersten Periode gebeten, die 0,06%ige Mundspllsung zu nutzen,
wobei fr die zweite Gruppe keine Intervention whrend
der ersten Periode vorgesehen war. In der zweiten Periode
wechselte dies. Um Residualeffekte, zum Beispiel Langzeit- oder berhangeffekte der Chlorhexidin-Mundspllsung zu vermeiden, wurde die Wash-out-Phase mglichst
lang gewhlt [2, 12, 31, 38].
Als Testprparat diente die 0,06%ige ChlorhexamedZahnfleisch-Schutz Mundspllsung (plus 250 ppm Natriumfluorid; Fa. GlaxoSmithKline Consumer Healthcare GmbH
& Co.KG, Bhl, Deutschland).
Alle studienbedingten Manahmen wurden von derselben Person an acht kontinuierlichen Untersuchungszeitpunkten im Abstand von jeweils 35 Wochen durchgefhrt. Zuvor
erfolgte eine eingehende Kalibrierung und Unterweisung der
Untersucherin, insbesondere die Kalibrierung des Geruchsinns mittels so genannter Sniffin Sticks [11]. Die Patienten
wurden mglichst morgens untersucht. Die folgenden Messungen und Parameter wurden durchgefhrt und untersucht:
die mastikatorisch stimulierte Speichelflierate (in ml/min,
stimuliert durch zweimintiges Kauen eines Paraffin-Krpers);

Statistische Auswertung
Die Auswertung der erhobenen Daten erfolgte mit Hilfe des
statistischen Programmpakets PASW Statistics 18.0.1 (SPSS
Inc., Chicago, IL, USA). Die statistische Auswertung oblag
den besonderen Voraussetzungen des Cross-Over-Designs
[19, 20], speziell die Bestimmung von Chlorhexidin-Behandlungseffekten (Differenzen der jeweils letzten Untersuchungszeitpunkte der zwei Perioden pro Gruppe) und mglicher Residualeffekte einer Untersuchungsperiode (Mittelwerte aus der Summe der jeweils letzten Untersuchungszeitpunkte der zwei Perioden). Die Datenstze wurden mittels
Mann-Whitney-U-Tests fr nicht parametrische Daten miteinander verglichen. Den Tests wurde eine Irrtumswahrscheinlichkeit von = 0,05 zugrunde gelegt.
Um herausfinden zu knnen, ob Mundgeruch entstand
und inwieweit sich die anderen klinischen Parameter vernderten unabhngig von der Chlorhexidingabe wurden fr
jeden klinischen Parameter zu jedem der acht Untersuchungstermine die Mittelwerte, die Standardabweichung und die
95%-Konfidenzintervalle (oberes und unteres Intervall) berechnet.
Der Fragebogen wurde ebenfalls fr alle Probanden
(n = 31), das heit nicht Gruppen-differenziert, rein deskriptiv ausgewertet.
Ergebnisse
Klinische Parameter
Die Ergebnisse der Untersuchungen sind in den Tabellen
1 bis 7 dargestellt.
Die niedrig dosierte Chlorhexidin-Mundspllsung
zeigte in der vorliegenden Studie bei Verwendung eines Signi-

Questionnaires
The questions presented in Tables 8a and 8b and the distribution of answers represent a selection of the questions
contained in the study questionnaires. These are chosen
primarily to reflect the oral health-related quality of life of
the patients. The majority of patients marked hardly ever
in relation to problems with sense of taste, eating, daily activities (Table 8a) and impairment in terms of shyness or
insecurity (Tables 8b) due to wearing the Invisalign aligners. During the first three months, patients experienced
problems with the pronunciation of certain words (Table
8a) or pain sensations (Table 8b), responding on average
with once in a while or hardly ever. These pains were
described especially during the first 2 to 3 days of a new
pair of aligners and diminished during the subsequent
course of the treatment.
In addition, the questionnaires recorded data on teeth
cleaning and oral hygiene habits, subjective belief in monitoring their own dental health, utilization of dental services,
motivation to persist with the orthodontic treatment with Invisalign and perception and description of oral malodor and
dry mouth.
The patients displayed a growing sense of personal responsibility for their own dental health during the course of
the treatment, in which advice from others, especially their
dentist, played a major role. In this context it should be noted
that 61% of the subjects learnt about Invisalign from their
dentist. 90% of the interviewees had been consulting the
same dentist for years and 77.5% attended regular dental
check-ups. In this study 74% of those questioned stated that
they had undergone the orthodontic treatment for esthetic
reasons. 23% regarded function as the primary reason for
their decision.
The number of subjects who cleaned their teeth three or
more times a day more than doubled during the course of the
study (from 11 to 28 patients). The mean time spent cleaning

teeth was also increased: at the last control visit the majority
of the study participants stated that they cleaned their teeth
for about three minutes (18 subjects compared with seven at
the start of the study).
Discussion
Clinical Parameters and Oral Health
There were no elevated Halimeter readings at any stage.
The three halitosis parameters improved during the course
of the Invisalign treatment. Comparison with similar Invisalign studies is not possible given the current state of
research.
Furthermore, the subjects displayed no signs of gingivitis despite all-day coverage of the attached gingiva by the
aligners. Nevertheless, an improvement in the mean gingival
index from 0.04 to 0.000 was described. These values are very
low in comparison with studies by the groups of Miethke et
al. [25, 26]. In the study by Miethke & Brauner [25] the mean
gingival index improved from 0.71 to 0.46 after 3 months of
Invisalign treatment (without intervention, with constant
oral hygiene instructions). By comparison, such results
seemed statistically significantly lower than in patients with
a fixed lingual appliance, where the mean gingival index for
the three check-up sessions was relatively constant at 1.00.

Darber hinaus wurden Daten zu Zahnputz- und Mundhygienegewohnheiten, zur subjektiven Kontrollberzeugung
der eigenen Zahngesundheit, zur Inanspruchnahme zahnrztlicher Dienste, zur Motivation hinsichtlich der kieferorthopdischen Behandlung mit Invisalign und zur Empfindung
und Beschreibung von Mundgeruch und Mundtrockenheit
durch die verwendeten Fragebgen ermittelt.
Die Patienten wiesen ein im Therapieverlauf wachsendes
Gefhl der Eigenverantwortung bezglich der eigenen Zahngesundheit auf, wobei Ratschlge anderer, insbesondere des
Zahnarztes, eine magebende Rolle spielten. In diesem Zusammenhang ist anzumerken, dass 61% der Probanden durch
ihren Zahnarzt von Invisalign erfuhren. 90% der Befragten
konsultierten seit Jahren denselben Zahnarzt und 77,5% gingen regelmig zu zahnrztlichen Kontrolluntersuchungen.
In der vorliegenden Arbeit gaben 74% der Befragten an, aus
sthetischen Grnden die kieferorthopdische Behandlung
durchfhren zu lassen. 23% sahen die Funktion im Vordergrund ihrer Entscheidungsfindung.
Im Verlauf der Studie stieg die Anzahl der Probanden,
die dreimal oder hufiger pro Tag die Zhne putzten, um
mehr als das Doppelte an (von 11 auf 28 Probanden). Die
durchschnittliche Dauer des Zhneputzens konnte ebenfalls
gesteigert werden, wobei zur letzten Untersuchungssitzung
die Mehrheit der Studienteilnehmer angab, zirka drei Minu-

not available for routine use in the early nineties. With regard
to the familiarization period, Nedwed & Miethke [30] found
in their study on the Invisalign system that 83% of the patients examined adapted to the treatment within a week. 35%
of the patients had no pain at all when wearing the aligners
and 54% experienced only slight pain within 2 to 3 days after
insertion of a new aligner. This period of a few days could
basically be explained by the controlled prestressing of the
semi-elastic aligners. Impairments, such as limitations in everyday conversation, were hardly ever recorded. 89% of the
patients stated after 3 to 6 months that they were satisfied
with the progress of their treatment and 96% said they were
highly motivated for the remaining treatment period.
The patients in the present study reported almost no reduction in oral-health-related quality of life and social integrity during the first 8 months of the Invisalign treatment.
One criticism worth noting is that the option of never was
not chosen as an answer at any stage. However, given the
great acceptance and satisfaction of the questioned patients,
coupled with their sensitivity to very small oral-specific
changes, hardly ever with reference to impairment would
seem equivalent to not significant reduction in quality of life.
Slight pain was experienced for a relatively long time in
this connection, compared with the above-mentioned literature. Once again the subjects experienced pain classified as
hardly ever noticeable within the first 2 to 3 days after insertion of new aligners. Impairments in everyday life were
hardly ever noticed by the great majority of subjects (21
31 of 31 subjects) over the whole study period. A feeling of
insecurity or shyness was hardly ever perceived by the majority, in the same way as impairment of eating or the sense
of taste.
Pronunciation of certain words was slightly affected, especially within the first 3 months (T3 and T4; Table 8a).
These initial problems abated during the course of treatment. By comparison, Fritz et al. [8] pointed out that tongue
function was impaired in two-thirds of patients during fixed
lingual therapy, and irritations and speech problems such as
lisping were reported.
Strong belief in monitoring personal dental health and
frequent utilization of dentistry services were established
among the respondents. Generally speaking, the regularity
of visits to dentists has increased in Germany according to
results of DMS IV. This trend should be greatly welcomed
for reasons of primary and secondary prevention of oral diseases [24]. It was not surprising that very good, subjective
belief in monitoring their own dental health was found in the
patient population studied here, who were very conscious of
oral health.
At the end of the study 84% of the subjects questioned
were very satisfied with the result of the treatment and
