height of chimney. Reccommended encapsulation by
Forest as safe access to access internal chimney full
height would require specialist team). Client made
aware of situation, no instruction given.

**Identified by Riverside as AIB - removed. Adams


incorrectly identified all 4no. rooms on 3rd floor
mezzanine to be AC ceiling panels, only 1 no room
found to have AC panels (3.04). During works to
remove AIB, further traces found by Forest and
Riverside on nails to rafters behind supalux (Samples
by Riverside confirmed as AIB. 08/02/16). AIB nails and
further 2 AIB sheeting to rafter found and removed in
early Feb following additional client instruction.

(Panels incorrectly labelled by Adams as
Asbestos).

Noted that scaffolding/enclosure was erected and ASB


5 issued for license before found to be incorrectly
labelled. ST.02 was on adams report but not labelled in
situ. ST.01 was not on Adams report but labelled as
Asbestos in situ - As had been similar confusion with
report, Forest & Riverside proceeded as if was AIB
panels presuming ST.01 to be the same as ST.02.

External item - removing mastic would risk water


ingress to building. Item to be moved to external
package (removed during demo of tanks takes place as
holds tanks together) when scaffolding/access to roof
area is ready. (ALL external asbestos works to be
formed in separate works package)

embedded in timber). Indecision over final floor level
and if timber boards should be salvaged. Forest
suggested temporary encapsulation with matting (was
never actioned, Forest never submitted price for
works).

(revealed AIB debris) - Removed following Client
Instruction.
