0 Quality issue /problem

Since 2013, various car manufacturers have issued recalls for cars that use Takata airbags,
after it was found that they may in some occasions, deploy explosively and send fragments
into the cabin. This has caused more deaths and injuries so far. This case make off the
largest automotive recall in U.S. history. More than 100 people have been hurt by the
inflators, which can explode with too much force, blowing apart a metal canister and sending
shards into drivers and passengers.
Many of drivers more around the world have been injured affected of exploding
inflator.Because of that the death because of defect Takata airbags had spurred millions of
airbags recalls worldwide.That including 24 million vehicles containing 28 million
potentially inflators in US. According to the National Highway Traffic Safety Administration,
less than 8 million airbags have been replaced due of defective Takata airbags before.
While issue of defective Takata airbags have been headlines ,that also give influence on
other companylike Nissan company .Nissan ordered the recall of nearly 4 million vehicles
sold worldwide because of defect on sensor that result in airbags failing to deploy when in
crash happen .Takata also faced the problem on replacing part of their defective airbags
.Takata also has been make a high effort to delivery necessary airbag inflator to cover all the
recall .Some of owner of the vehicle give complaints on have to waited for dealer to get the
required part to replace the defect of airbags .

2.0 Root cause

The Independent Testing Coalition, made up of investigators from 10 automakers and led by
David Kelly as the head of the National Highway Traffic Safety Administration, shows the
results of its investigation into Takata exploding airbag inflators. The use of ammonium nitrate as
a propellant was one of the causes for the ruptures and explosions. It can destabilize over time

and violently rupture the metal casing, or inflater, sending metal shards flying inside the
vehicle.It also may caused by moisture seeping into airbag inflators which use ammonium
nitrate-based propellants that lack a chemical drying agent.

Airbags rupture when phase stabilized ammonium nitrate in the airbags do not have a
moisture-absorbing desiccant. When moisture is present in long term exposure that repeated on
high temperature cyling and airbag inflator assemblies that do not adequately prevent moisture
intrusion. Moisture can cause ammonium nitrate to destabilize, leading to explosions during
crashes. Takata airbags more likely to explode after long-term exposure to heat and moisture.
The moisture, compounded with changes in temperatures, can make the propellant unstable and
more liable to explode and rupture its metal container..

High humidity also the possible causes of faulty of Takata airbags .High humidity can
exposure to moisture that may seep and absorbed by airbag inflator. Which that will develop
higher internal pressure for a long period of time. Higher pressure can causes the airbags to
explode in accident .Exposure to high humidity and inflators assembly cannot prevent moisture
intrusion .Failure to protect chemical from moisture will happen when propellant used in inflator
was exposure to humid climates. High humidity also make airbags to inflate with too much force
and hurl shrapnel at drive and passengers. Exposure to humidity can make the ammonium nitrate
to combust violently and rupture the inflator when deploy in a crash. Fatal injuries more likely
happen in driver side ones though it is unclear whether that is because driver always present.
Beside that driver and passenger side car that have problem inflaters will causes greater danger.

Other problem are came with the airbag construction .Airbag can explode and overinflate if
the inflators are overpowered.If crash sensors that active the airbags system not optimally located, the
airbag would not inflate in time to protect .Malfunction can happen and cause harm to occupant when
improper folding of the airbag.

Airbags react with chemical that fills the airbag rapidly with gas .Ammonium nitrate is the chemical
used in inflator of airbags. The unstable chemical may create too strong a force when ignited, and become
hazardous.

Internal investigation made by Takata had found that chewing gum, bad welds and also rust
in airbags is the fault for the rupture of airbags.This can cause inflators failed to function .

3.0 Corrective action

Because of the dangers of its exploding airbag inflators, Takata has take action on recalled
the units. Takta make the recall a massive endeavor .Its limited to cars in high humidity areas
.This is because malfunction of inflators are maybe due to rust on the inflators caused by
humidity. Owners of the car that use Takata airbags will get notice from Takata airbags to replace
the airbags especially . Customer has to take take immediate action for older vehicle.
Manufacturer will send warnings sign for drivers to watch it. The notice will also include
information on how the manufacturer is going to handle the recall.As a customer also have to
make an appointment with dealer to repaired your airbags .

Action that consumer that have a faulty Takata airbags is they have park their car and never
touch it again until it's fixed. They have to send to manufacturer and ask to replace their airbags
and not use the car until have done repaired it .They also had to send their cars to Takata
manufacturer to replace the inflator .Other than that, consumer has to minimizing the uses of car
and not to allowes passenger to ride in affected seats.

Takata have to listed the recall cars by the National Highway Traffic Safety Administration
must be repaired for free. Takata company are responding to this safety risk by recalling all
vehicles that have these specific air bags for the customer safety. So as customer has to bring
recall letter to show to dealer .The listed cars to be recall has to send their cars for repair .It is for
your safety .Replacement of Takata airbags help to prevent of death or injuries of drivers or
passenger . Recalled the vehicle that using Takata airbags is given priority to cars that are
generally from model years 2008 or older, and that were originally sold or registered in areas of
high absolute humidity.

Takata. To date, the Company has produced. Currently over 66% of replacements kits
include non-ammonium nitrate inflators manufactured by other suppliers. Takata has
increased production capacity to 1.5 million replacement kits per month, up from 350,000 per
month in December 2014.

4.0 Prevent from recurring

The step for prevent from recurring of rupture airbags is consumer should not try to buy
replacement parts on their own .It is because if consumer take action on their inflators may effect
of deploy more of airbags. Customer have to be cautious when driving a vehicle. Although the
airbag may spontaneously trigger itself, most cases happen during accidents. So if you avoid
accidents, you greatly reduce the risk of injury from an exploding Takata airbag. Customer also
need to be alert when have listed by Takata to be recalled .Customer has to send their car as soon
as possible to replace the airbags before it rupture and cause accidents

Takata also have take their action on making a safe airbag is to use propellants that
stable .Takata have to ensure the airbags their produce are safety for consumer with high quality
airbags.So company had to spend money to develop their company and make research on
improving their quality on airbags to ensure that safety of consumer not at risk.
On the other hand, to protect public from unreasonable risk to safety is Takata company
should accelerate the phase out schedule for non desiccated ammonium nitrate-based inflators
and to create a phase-out schedule for desiccated ammonium nitrate-based inflators. According

to the consent order terms, Takata has until 2018 to cease production of all non-desiccated
ammonium-nitrate inflators and until 2019 to prove that ammonium nitrate is safe within its
most-current products. While Takata agreed to stop entering new contracts for desiccated
ammonium-nitrate inflators, there is no production cap on those inflators.

Millions of additional vehicles with Takata ammonium nitrate inflaters still on road and
have to been recalled at the end of 2018 for safety .Takata inflater using a dessicant ,a chemical
that help reduce moisture to reduce ruptures. Manufacturer also not have to rely on the chemical
for produce inflator because it is more risk for the customer safety .Company should not
depending on chemical on inflator because of safety .
The important action to do is the drivers has to make they safety when driving .When
driving a vehicle customer has to wear safety belts . It can prevent injuries when airbags
deploy.Airbags are designed to supplement seat belts in an accident, not replace them.If you are
not wearing the seat belt will effect on die in accident when an airbags was deploy.As drivers
have to sit far away as you can from the steering wheel or passenger side dashboard.It is because
distance from the airbag is the most important risk factor in airbag safety.Other than that drivers
has to try keep keep an adjustable steering wheel tilted down in a level or parallel position.

Takata airbags has lack of quality control .So they have improve their quality of airbags
with doing testing on the returned inflators.This can help to show that exposure over a period of
many years to a climate of persistent heat and high absolute humidity that was the primary
factors in the small number of inflators that have malfunction .
