Now comes the Accused, Name of Accused, and for his Motion for Substitution of Judge, states
as follows:
1. The movant is the defendant in the above-entitled cause, now pending before this Court.
2. The movant represents that he fears he will not receive a fair and impartial trial before the current
trial judge, the Honorable Judge Vincent Lopinot, for the reason that said Judge is prejudiced against the
defendant.
3. This Motion is timely filed, within ten days after this cause was placed on the trial call of the
aforesaid Judge, pursuant to 725 ILCS 5/114-5(a).
WHEREFORE, the defendant prays that this Court grant a substitution of judge and that this cause be
transferred to the Assignment Judge for reassignment.
