The literature states that the proteolytic enzyme papain in PapacarieTM interacts
with partially degraded collagen in the necrotic tissue of carious lesions, causing
additional softening of this tissue14. This proteolytic action likely occurs only in
the necrotic tissue, as sound tissue contains alpha-1-antitrypsin, which is an
antiprotease that impedes the action of proteolytic enzymes (Santos, et al,
2015).
Recent studies investigated the influence of various growth factors in scar and
keloid formation. TGF-b and PDGF have been shown to play an integral role in the
formation of hypertrophic scars and keloids. The majority of cells involved in
wound healing express TGF-b in an inactive form that strongly promotes the
chemotaxis of fibroblasts to the site of injury. Moreover, this growth factor plays
a critical role in fibroblast proliferation and collagen production. 27 When wound
repair is completed, the activity of TGF-b is normally turned off. In keloidal tissue,
TGF-b is overproduced and poorly regulated through normal autocrine signaling
mechanisms. At the same time, keloid fibroblasts have greater numbers of
growth factor receptors and respond more intensely to growth factors such as
TGF-b and PDGF. Less synthesis of molecules that promote matrix breakdown
(e.g., MMPs) may also explain the lack of scar regression seen in keloids
Several MMPs have been shown to mediate the breakdown of type I and III
collagen, the most abundant types of collagen in the skin ECM(66). Specifically,
MMP-2 and MMP-9 activity persists after wound closure and seems to play a
potent role in the remodeling process (Gauglitz et al,2011)
The extracted papain showed the MMP-2 stimulatory activity papain has higher
proteolytic activity and gives more amount of the secreted collagenase in the
fibroblast. This study has suggested that the extracted papain from plant can be
further developed for the treatment of keloids and hypertrophic scars.
demonstrated that papain has higher proteolytic activity and gives more amount
of the secreted collagenase in the fibroblast (Manosroi et al, 2014).
