2. Two coloured photographs taken recently and of prescribed size (5 cm x 6 cm). These
photographsshouldindicatenameofcandidateincapitallettersandattestedontheback.
3. Last Admit Card is to be returned with present application. The Admit Card for the present
examinationwillbeissuedonlywhenthelastAdmitCardhasbeenreturned.Incasetheadmit
cardhasbeenlost,penaltycouldbeimposed
4. Incasethecandidateisrepeatingtheexaminationafteraperiodofmorethanthreecalendar
years,he/sheshouldapplyontheapplicationformprescribedforFRESHCANDIDATESand
attachalltherequireddocumentsagain.
5. A candidate who does not appear in TOACS / Clinical examination of the same session after
passingtheoryexaminationwillloseonechance.Onlytwooutofthenextthreeconsecutive
chanceswillbeavailabletosuchcandidates.
6. Theory examination is conducted at Karachi, Hyderabad, Quetta, Multan, Lahore, Faisalabad,
Rawalpindi,Bahawalpur,Islamabad,Peshawar,Larkana,Abbottabad,Nawabshah.TheCollege
canholdtheoryandclinicalexaminationsinoneormorecitiesofthecountrydependingonthe
numberofcandidatesandlogisticfacilitiesavailableinacityirrespectiveofthechoicegivenin
theBoxabove.
